| Object: The purpose of this study was to compare the short-term treatment surgical outcomes between robotic-assisted versus laparoscopic surgery in 192 patients with endometrial cancer who were admitted to the Gansu Provincial Hospital from January 2014 to March 2018.Methods: The clinical data of 192 patients with endometrial cancer(EC)from January 2014 to March 2018 in Gansu Provincial Hospital were retrospectively analyzed.There were 112 cases in robotic-assisted surgery group(RS)and 80 cases in laparoscopic surgery group(LS).Demographic data and general clinical data were recorded.we have analyzed surgical outcomes including operating time,estimated blood loss,length of hospital stay,total cost,day of Indwelling catheter,lymph node dissected,drainage tube indwelling time,anal exhaust time by multiple linear regression analysis,and analyzed the rate of conversion to laparotomy and transfer to ICU(intensive care unit)by multivariate logistic regression analysis.The multivariate regressions were used to control the influence of the potential confounders.Results: There was a significantly different in operating time(95%CI=-57.26 to-5.57,P=0.017),length of hospital stay(95%CI=1.76 to 4.22,P<0.001),day of Indwelling catheter(95%CI=-0.97to-0.63,P<0.001),lymph node dissected(95%CI=0.52 to 5.35,P=0.017),anal exhaust time(95%CI=-0.08 to 0.47,P=0.005),and there was no significant difference in estimated blood loss(95%CI=-5.26 to 77.04,P=0.087),total cost(95%CI=-3155.90 to 2959.80,P=0.950),drainage tube indwelling time(95%CI=-0.65 to 0.45,P=0.721).In the multivariate logistic regression analysis,there was no significantly different in rate of transfer to ICU(95%CI= 0.31to13.30,P=0.461),and conversion to laparotomy(95%CI=0.00 to 2.57,P=0.110).Conclusion: Compared with traditional laparoscopic surgery,Davinci robotic-assisted surgery has the advantages of shorter operating time,fewer lengths of hospital stay,more total lymph node dissection and lower incidence of short-term complications after operation.The short-term clinical effect is superior to the traditional laparoscopy,and it is worthy of extensive clinical application and popularization. |
