Background and purposeThe main function of Pit and fissure sealant is to prevent dental caries,which is a common and effective treatment,and has been widely used in many countries at home and abroad.The incidence of caries among children in China is relatively high.In 2017,the ministry of health conducted the third national epidemiological survey on oral health.According to the results,the rate of caries among children aged 5 years reached 70.9%,far higher than that of permanent teeth of children aged 12 years(34.5%).Caries of deciduous teeth is one of the diseases that seriously endanger children's oral health.At present,Pit and fissure sealant technology has been widely used in the prevention of permanent tooth decay in China.In clinical practice,resin materials are mainly used for the closure of permanent alveolar groove,which has the advantages of simple operation,good fluidity,good permeability and low shedding rate.However,resin type socket and groove sealant has a high requirement for moisture separation,and the acid-eroded tooth surface is easy to fall off after being polluted by saliva,so it is not easy to achieve the ideal efrect when used for deciduous teeth.Therefore,it is urgent to find a kind of material suitable for the closure of the pit and fissure.Represented by by Fuji ? glass ion class nest ditch sealing material,a short operation time,occasionally wet insulation bad smaller efrect on the filling performance,etc.The use of dental Pit and fissure sealant is increasing.This study firstly made an epidemiological survey on the caries of the second molar-tooth of children aged 4?6 years old in zhangdian district of zibo city,which can enrich the data information and provide a better follow-up study to provide strong data support.Combined with the region's specific situation,screening of three groups of different agencies,in view of the different groups choose corresponding to a kind of clinical common resin class blocking agent,a kind of hydrophilic material wet bonding nest channel blocking agent,a nest glass ion channel blocker,implement nest groove closed operation,conducted by the inspection and operation are completed in the school.By nest channel blocker off rate and international caries disease detection and evaluation system of three different kinds of litter sealants in closed after 3 months,6 months,9 months of closed effect is analyzed,and their effect and from the perspective of economics to three sets of materials to carry on the preliminary evaluation,so as to achieve a better design consistent with the actual situation in our country.Materials and methodsThe first part is the investigation and analysis of second molars caries of children aged 4?6 in zibo cityWith the method of cluster system sampling,7 kindergartens in zhangdian district of zibo city were randomly selected.A total of 728 children aged 4-6 years old were selected,all of whom had local household registration in zibo,including 402 boys and 326 girls.With reference to the world health organization(who)issued by the oral health survey basic method,and according to our country the third national epidemiological sampling survey of oral health programme content requirements,by zibo oral hospital children,s dental professional doctor,using the unified configuration of dental flow check chair,portable lighting,and disposable oral cavity instruments for children,s teeth with caries status inspection and record,through standard conformance test prior to the survey unified training(Kappa value>0.8),and the two other than oral professional staff entry examination results respectively,and then adopt statistical software for data processing and analysis.Chi-square test was used to compare the caries rate and other indicators,and the test level was selected as alpha=0.05.The second part compares the effects of different types of Pit and fissure sealant materials in the prevention and treatment of primary cariesAccording to the requirements of this experiment,unified inclusion and exclusion criteria are formulated.The selected 300 school-age children were randomly divided into groups(group A,group B and group C).Group A used resin sealant,group B used resin sealant,and group C used glass ion sealant to seal the selected molars.All examinations and procedures were performed on campus by two pediatric stomatologists.Using ICDAS ?,sealants retention two indicators of three different kinds of materials in a closed after 3 months,6 months,9 months of their effect and closed effect is analyzed.The third part is the preliminary economic evaluation of different pit and groove sealing materials for the prevention and treatment of cariesA preliminary economic evaluation through the following two aspects,first,the choice cost effect analysis than in this respect,as an important reference index,combined with a variety of operation process of the selected materials for calculation,three groups of need how many size is obtained only cost data values,such as additional also can calculate the transportation and labor data values,based on precise analysis.Select the "prevent second milk teeth have caries(ICDAS-?classification of 2)or more teeth number,to evaluate the effect of indicators.To compare different materials to prevent 1 second milk molars of caries(ICDAS-?grade 2 or more)needed for input costs.In addition,there were 2 pediatric stomatologists involved in all operations and examinations.The two subjects were calculated to understand the specific time when a tooth was closed in the pit and fissure,and three times for different materials.By comparing the data,different time periods could be understood.The results1.The first part of this study investigated and analyzed the caries of 728 children aged 4?6 years old in zhangdian district of zibo city.For the subjects with different genders,there was no statistical difference in the caries rate of the second molars,while the caries rate of the second molars in the lower molars was slightly higher than that in the upper molars,but the difference was not statistically significant.2.The nine-month observation on the sealing and caries prevention of three groups of different pit and groove sealants in the campus group prevention and control mode shows that the retention rate of glass ion sealant is higher than that of wet bond fluorine-containing resin sealant and common,fluorine-containing resin sealant;The effect of glass ionomer sealant is better than that of wet bond sealant and common sea ant.3.The primary effect ratio is 11.66,13.05 and 10.57,among which the lowest is glass ion socket and groove sealant group,and the highest is wet bond fluorine-containing composite resin group.It can be found that the time spent in the group of glass ion pit and groove sealer was significantly lower than that of the other two groups.Conclusion1.The second molars of children in zhangdian district of zibo city have more serious caries,and the disease starts earlier,with a higher caries rate.Relevant departments need to pay attention to this aspect.2.Glass ion socket and groove sealant has a better prevention effect on caries of deciduous teeth than resin socket and groove sealant.Moreover,the former has many advantages,such as simple operation,and can be well applied in the campus and play its due role.However,it cannot be directly applied at present and needs a long period of clinical observation.
