Objective:Taking the pregnant women who had a regular antenatal examination and delivery in a hospital in Shenyang as the subjects of the study.To monitor the blood flow parameters of the UtA(Uterine arteries)of pregnant women during early,middle and late pregnancy and the fetal UA(umbilical arteries)during middle and late pregnancy by CDFI(Color Doppler flow imaging).The high-risk pregnancy in the study includes HDP(Hypertensive disorder of pregnancy),GDM(gestational diabetes mellitus),FGR(Fetal growth restriction)and Amniotic fluid anomaly.To compare the difference of blood flow parameters of UtA and UA in different pregnancy between the high-risk pregnancy group and the normal pregnancy group and analyze the relationship between abnormal blood flow of uterine artery and fetal umbilical artery of different pregnancy and pregnancy outcome,so we can understand the condition of uterus-placenta-fetal circulation,effectively monitor the intrauterine safety of the fetus and provide guidance for clinical to treat the conditions timely which was endangering mother and child.Methods:170 high-risk pregnant women and 150 normal pregnant women who had regular antenatal examination in a hospital in Shenyang from July 2017 to July 2018 were collected by convenient sampling.To collect the basic data of pregnant women and their newborns.uterine artery,in early pregnancy(10-14 weeks),mid pregnancy(20-26 weeks),late pregnancy(28-36 weeks)and the blood flow parameters of fetal umbilical artery and the growth and development of fetus in mid pregnancy and late pregnancy were analyzed.The diagnosis standard of high-risk pregnancy is based on the eighth edition of Obstetrics and Gynecology published by people's medical publishing house.The database was built by Epidata software and statistical analysis was performed by IBM SPSS 24.0 software.The quantitative data were expressed by x?±s,and analysis the data for normality,the t-test was used for comparative analysis with the normal distribution data.The count data was taken by chi-square test,and the test standard was set to ?=0.05.Outcome:1.The PI,RI,S/D of uterine artery group in the middle and late pregnancy with HDP were higher than the normal pregnancy group,the difference was statistically significant(P < 0.05).The rate of uterine artery notch in the early stage of diastole during the whole pregnancy was higher than the normal pregnancy group,and the difference was statistically significant(P<0.05).and the RI,S/D of fetal umbilical artery in the middle pregnancy and the PI,RI,S/D in late pregnancy of fetal umbilical artery were higher than the normal pregnancy group,the difference was statistically significant(P<0.05).2.The PI,RI in the middle pregnancy and the PI,RI,S/D in late pregnancy of uterine artery with GDM were higher than the normal pregnancy group,the difference was statistically significant(P<0.05).The rate of uterine artery notch in the early stage of diastole during the whole pregnancy was higher than the normal pregnancy group,and the difference was statistically significant(P<0.05).The PI in the middle pregnancy and the PI,RI,S/D in late pregnancy of fetal umbilical artery with GDM were higher than the normal pregnancy group,the difference was statistically significant(P<0.05).3.The PI,RI,S/D of uterine artery in middle and late pregnancy of pregnant women with FGR were higher than those in normal pregnancy group,and the difference was statistically significant(P<0.05).The rate of uterine artery notch in the early stage of diastole during the whole pregnancy was higher than the normal pregnancy group,and the difference was statistically significant(P<0.05).The RI in the middle pregnancy and the PI,RI,S/D in late pregnancy of fetal umbilical artery with FGR were higher than the normal pregnancy group,the difference was statistically significant(P<0.05).4.The PI,RI and S/D in the middle and late pregnancy in the pregnant women with amniotic fluid abnormal were higher than the normal pregnancy group,and the difference was statistically significant(P<0.05).The rate of uterine artery notch in the early stage of diastole in early pregnancy was higher than the normal pregnancy group,and the difference was statistically significant(P<0.05).The RI,S/D in the middle pregnancy and the PI,RI,S/D in late pregnancy of fetal umbilical artery with amniotic fluid abnormal were higher than the normal pregnancy group,the difference was statistically significant(P<0.05).5.The incidence of adverse outcomes in the high-risk pregnancy group was significantly higher than the normal outcomes group.The cesarean section,premature delivery,fetal distress and fetal heart abnormalities in high-risk pregnancy group had a higher incidence than the normal pregnancy group,the difference was statistically significant(P<0.05).The weight of newborns in the high-risk pregnancy group was lower than the normal pregnancy group,and the difference was statistically significant(P<0.05).6.In high-risk pregnancy group,the PI,RI,S/D of uterine artery during the whole pregnancy and the PI,RI,S/D of fetal umbilical artery in the middle and late pregnancy of abnormal pregnancy outcome were higher than normal pregnancy outcome,the difference was statistically significant(P<0.05).The notch in the early stage of diastole of uterine artery in middle and late pregnancy of abnormal pregnancy outcome was higher than normal pregnancy outcome,the difference was statistically significant(P<0.05).Conclusion:1.The PI,RI and S/D of uterine artery in pregnant women and fetal umbilical artery of high-risk pregnant women were higher than normal pregnancy to different degrees,indicated that it may had affected the blood supply of the placenta and the growth and development of the fetus with the vascular resistance increasing.2.In early pregnancy,the PI,RI,S/D of the uterine artery was no significant difference between each high-risk pregnancy group and normal pregnant women.The incidence of early uterine artery diastolic notch was higher than that of the normal pregnancy group.So it has low sensitivitity to detect the high-risk pregnancy.In the middle and late pregnancy,the PI,RI,S/D the uterine artery and the incidences of the notch of early diastolic uterine artery in high-risk pregnant women were significantly different from those of normal pregnancy group.So it has high sensitivitity to detect the high-risk pregnancy.3.To monitor the blood flow parameters PI,RI and S/D of uterine artery and fetal umbilical artery and the early diastolic notch of uterine artery of pregnant women by color Doppler can reflect the flow status between uterus placenta and fetus placenta.It can provide an effective reference for the monitoring of adverse pregnancy outcome in high-risk pregnancy.
