| Objective:To compare and analyze the clinical effect between 2.2mm incision and traditional 3.2mm coaxial incision cataract phacoemulsification in uncorrected visual acuity(UCVA)、best corrected visual acuity(BCVA)、angle of the anterior chamber(ACA)、anterior chamber depth(ACD)、volume of the anterior chamber(ACV)、intraocular pressure(IOP)、corneal endothelial count、surgery induced astigmatism(SIA)and so on.Methods:Choose from January 2015 to October 2015 cataract patients cured in our hosp-ital ophthalmology department,total of 100 cases of eye,except patients with p-terygium or underwent pterygium resection,history of ocular trauma,glaucoma,retinal pathological changes.They were randomly divided into A and B groups.Patients of group A were treated with 2.2mm incision cataract phacoemulsificat-ion operation,while group B with 3.2mm incision.Through observation of the two groups patients with postoperative one day,one week and one month,we recorded the visual acuity,intraocular pressure,anterior chamber angle,anterior chamber depth,anterior chamber volume,corneal endothelial cells counts,corneal astigmatism and so on.Analysis the data by use of statistical software with chi-square test and t test.Results:The visual of the two groups patients improved than pre-operation,UCVA and BCVA after operation in the first day、the first week and the first month,the group of A compared group B,have no statistical significance(P>0.05),but the visual of group A better than group B.Intraocular pressure after operation decr-eased than preoperative in both group A and group B.IOP after operation in the first month had no statistical difference with preoperative IOP(P<0.05)in Group B(or group A).After operation of the first day,the first week and the first month,IOP of group A respectively are 15.29±3.87mmHg、14.63±3.58mmHg、14.06±3.01mmHg,IOP of group A respectively are 15.73±4.83mmHg、15.40±3.04mmHg、13.87±2.67mmHg.Two groups had no statistical significance in sa-me time points after operation.Anterior chamber angle(ACA)after operation in the first day,the first week and the first month of group A respectively are 44.76±5.21°、45.23±7.46°、45.97±4.95°,same in group B are 44.10±4.37°、45.04±8.79°、46.83±5.94°,there was no difference between two groups in statistics at the same time points after operation(P>0.05).The postoperative ACA at different time of two groups increased than preoperative ACA and had statistical signifi-cance(P<0.05).Anterior chamber depth of postoperative 1st day,1st week after operation,postoperative 1st month,A group of patients were 2.84±0.63mm、3.01±0.59mm、3.09±0.49mm,group B were 2.76±0.54mm、2.94±0.67mm、3.11±0.57mm.There was no statistically significant difference at the same time points in the two groups(P>0.05).Post-operative anterior chamber depth are deeper than preoperative ACD in both group A and group B.Postoperative 1st month compa-red with before operation,the difference was statistically significant(P<0.05).Volume of the anterior chamber of postoperative 1st day,1st week after operati-on,postoperative 1st month,A group of patients were 140.35±32.47mm~3、151.26±29.30 mm~3、153.05±30.89 mm~3,group B were 139.07±24.20mm~3、152.6 0±4.79mm~3、154.73±26.59mm~3.There was no statistically significant difference at the same time points in the two groups(P>0.05).The first week、the first month of post-operative ACV was increased than preoperative ACV,the difference had statistical significance(P<0.05).Postoperative 1 day、1 week and 1 month,A group of corneal endothelial c-ell count were respectively 2125.19±140.65/mm~2、2013.29±145.43/mm~2、1999.09±139.52/mm~2,group B of corneal endothelial cell count were respectively 2141.33±156.51/mm~2、2035.05±157.84/mm~2、2004.43±134.06/mm~2,the two groups compared with no statistical difference at the same time points(P>0.05).Group B(or group A)compared between preoperative and postoperative corneal endoth-elial damage had statistical difference(P<0.05).Group A of average corneal astigmatism after operation one day and one w-eek were 1.07±0.57D and 0.83±0.51D,while Group B were 1.58±0.75D and 1.19±0.75D,there were statistically significance between the two groups at the same point time(P<0.05);average corneal astigmatism after operation one month of group A was 0.71±0.43D,while Group B was 0.74±0.47D,no statistical differen-ce between the two groups.Group A of surgery induced astigmatism after opera-tion one day and one week were 0.89±0.46D and 0.71±0.53D,while Group B we-re 1.37±0.51D and 0.99±0.42D,there were statistically significant between the two groups at the same point time(P<0.05);surgery induced astigmatism after operation one month of group A was 0.59±0.45D,while Group B was 0.67±0.46D,no statistical difference between the two groups.Conclusion:2.2mm cataract PHACO don’t have obvious advantage in corneal endothe-lial,intraocular pressure and the parameters of the anterior chamber when it co-mpared with 3.2mm PHACO,but it can reduce surgery induced astigmatism in early post-operative,so clinical effect is better than traditional phacoemulsific-ation. |
