Objective:Lumbar disc herniation has already become a common disease in orthopedic,most of the patients can present series of symptoms because of the roots of nerve or cauda equina been pressed,in severe cases can affect the patient's normal life.Now the treatments mainly include two types: conservative treatment and surgical treatment.With the development of medical technology and equipment become more and more perfect,the common conservative treatment,the mainstream position in the past,had been replaced by the Intervertebral foramen mirror,which with less trauma,less bleeding and faster postoperative recovery.But the intervertebral foramen mirror is higher requirements to the operation doctortechnology because of its complex and difficult disadvantages.This article through put the CT images into the digital Three-dimensional medical image processing software,with the help of the computer to deal with image information and get a Three-dimensional reconstruction.Make it can be intuitive and accurate observation under the three dimensional structure outstanding intervertebral disc position,extent,and adjacent to the dural sac,the relationship between spinal cord and nerve root.Through simulation approaches and operation for intervertebral foramen the puncture location selection,the needle depth of Angle in the direction of adjustment and provide theoretical guidance.To establish reasonable operation scheme to reduce the operation risk and difficult operation,reduce the number of intraoperative fluoroscopy,shorten the operation time and avoid the postoperative complications.Methods:Randomly selected the person with lumbar disc prolapse from our hospital,from June 2015 to June 2016.Adopts the principle of voluntary divided into A,B two groups.Patients of group A put its original CT and MRI image data into 17.0 digital Threedimensional medical image interactive processing system by setting a series of operations.Intuitive and accurate understanding of the outstanding intervertebral disc with adjacent nerve roots and dural sac and the location of the bony structure around it.To see the presence of abnormal nerve root development classification and so on carries on the observation position.By measuring its pathological changes of intervertebral foramen centering on the vertical axis and the loss of human body axis under different rotation angles corresponding transverse diameter and longitudinal diameter in the loss of the change of the length of the surface and coronary surface projection.Through a certain algorithm to calculate the puncture needle into different points and puncture point of work in the operation under the sleeve and intervertebral foramen cross sectional area to guide the operation.And group B patients with normal CT,MRI scanning imaging for diagnosis,intraoperative guidance,etc.Two groups of patients are operationed by the same doctor team and after operation tell them to stay in bed,wearing a waist exercise activities such as the same treatment.By comparing the two groups of patients in the number of intraoperative fluoroscopy,the effect of operation time,postoperative treatment and postoperative complications such as presence of differences.Using spss 20.0 software for statistical analysis of test.Comparative differences have statistical significance to verify the preoperative CT three dimensional reconstruction on the patients for intervertebral foramen mirror technology have guiding significance to the clinical.To provide guidance for clinical diagnosis and treatment of disease.Results:By comparing the number of A,B two groups of patients show that preoperative CT three-dimensional reformer on the patients more conventional pure line of lumbar vertebra CT on the operation time,number of perspective,have significant difference(P<0.05).But intraoperative bleeding,length of hospital stay,postoperative complications and postoperative visual analog pain score(VAS)have no significant difference(P>0.05).Conclusion:Apreoperative use of CT thin layer scanning to obtain image data,Then by Mimics software to establish three-dimensional model of lumbar spine corresponding segment of the high authenticity and accuracy.Through the model of the segmental spinal canal and internal morphological structure on qualitative observation and quantitative measurement.By Three-dimensional spinal model the bony structure and related soft tissue vivid,clear anatomical marks,And through the software process can be split to different organizational structure,enlarge,shrink,and rotated 360° all-round observation Can to the distinction between related structure in dyeing or transparent processing,And then more accurate concise and clear shows prominent segmental spinal canal structure and outstanding or hernia of intervertebral disc and nerve root,etc.The relationship between the surrounding soft tissue.To reduce the operation risk and difficult operation,reduce the number of intraoperative fluoroscopy,shorten the operation time and so on all has the important guiding significance.
