| ObjectiveTo explore the effect of left anterior descending coronary artery stenosis on the myocardial structure and function of the corresponding region by 256 layers of ICT.MethodsCollect our hospital 134 cases of patients from 2016-12 to 2017-12,divided into normal A group,the degree of stenosis of LAD 0-49% for the B group,more than 50% C LAD stenosis group,1,2,7,selected the 8 and 13 section of LAD absolute blood supply seg-ment,the original image reconstruction,Philips intellispace in protal for handling,process-ing end systolic left ventricular wall thickness(End Systole Wall Thickness,ESWT),end diastolic wall thickness(End Diastole Wall Thickness,EDWT),ventricular wall motion(Wall Motion,WM),ventricular wall thickening rate(Wall,Thickening%,WT%),and spss24.0 analysis data.Results1.Correlation and comparison of the classification of different non simple LAD stenosis and age and sexThere was a significant difference in sex composition between normal group and abnormal group.male age and LAD were positively related to the degree of stenosis,LAD related female age and the degree of stenosis was positively.The degree of LAD stenosis of men and women was correlated with age r=0.430,p=0.000 < 0.05,and the overall age was positively correlated with the degree of LAD stenosis.2.the correlation of the parameters of different non simple LAD stenosis groupsThe correlation analysis of EDWT,ESWT,WT,WM(S1,S2,S7,S8,S13)and the degree of vascular stenosis were analyzed.(1)The end diastolic wall thickness(EDWT): S2、S7、S8(p=0.011、0.011、0.014<0.05),S2、S7、S8 and the degree of stenosis was positively correlated.(2)The end systolic wall thickness(ESWT):S7(p=0.024<0.05),S7(ESWT)and the degree of stenosis was positively correlated.(3)The ventricular wall thickening rate(WT):S2(p=0.009<0.05),S2(WT)and the degree of stenosis was negatively correlated.(4)The ventricular wall motion(WM):S2、S8(p=0.000、0.047<0.05),S2、S8(WM)and the degree of stenosis was negatively correlated.3.The differences in the parameters of different non simple LAD stenosis groups(1)the end diastolic wall thickness(EDWT):S2,S7,S8(F=3.343,3.816,3.437),p<0.05,S2,S7,S8(EDWT)there are significant differences in the degree of stenosis of blood vessels.(2)the end systolic wall thickness(ESWT):S7(F=4.559,p=0.012),p<0.05,S7(ESWT)there are significant differences in the degree of stenosis of blood vessels.(3)the wall thickening rate(WT):S2(F=3.664),p < 0.05,S2(WT)there are significant differences in the degree of stenosis of blood vessels.(4)wall motion(WM): S2(F=13.785),p<0.05,S2(WM)there are significant differences in the degree of stenosis of blood vessels.4.Analysis of WT abnormal movement segmentWT < 30% as abnormal motion segment,normal segment count is 0,the abnormal stage of the count of 1,to calculate r=0.109,p=0.003 < 0.005,so we reject the null hypothesis,which is positively related to abnormal WT segment number and degree of stenosis.5.ROC curveSelect the S2 ROC curve,S2 wall motion(WM)value is 4.5mm,the sensitivity was 92.1%,specificity was 56.2%,Youden index was 0.483.6.The correlation between the degree of non simple LAD stenosis and EFGroup A was 63.79±4.88%,group B was 63.75±7.11%,and group C was 63.35±8.27%.R=-0.024,p=0.785 > 0.05,do not reject the original hypothesis,and there is no significant correlation between the degree of non simple LAD stenosis and EF.Homogeneity test of variance,p=0.022 < 0.05,compared with Tamhane ’T2,F=0.048,p=0.953 > 0.05,did not reject the original hypothesis,there was no significant difference in the degree of stenosis between different non simple LAD and EF.Conclusionthe left anterior descending coronary artery(LAD)with different stenosis degree is positively correlated with age in this study,and the proportion of male patients in LAD stenosis is higher than that in female patients.In different degree of non simple LAD stenosis,the thickness of S2,S7 and S8 increased with the increase of vascular stenosis in the end diastolic wall thickness(EDWT).In the end systolic wall thickness(ESWT),the S7 segment increased with the increase of vascular stenosis.In different degrees of LAD stenosis,the number of abnormal segments of the systolic wall thickening(WT)increased with the severity of non simple LAD stenosis.The S2 segment WM was used as an indicator of LAD stenosis,and the optimum threshold was 4.5mm.There was no significant difference between the non simple LAD stenosis and the ejection fraction(EF). |
