Preliminary Study On The Functional Of Evaluation Of Coronary Artery Critical Lesions And Prognosis Of Patients With MPI And FFR

ObjectiveTo compare the consistency and correlation between MPI and FFR in the diagnosis of myocardial ischemia in patients with coronary artery 50%?75%stenosis.To analyze the long-term prognosis between study group and control group.MethodsA retrospective analysis of 30 patients with coronary artery disease who underwent CAG and MPI in 2 weeks.CAG showed that 1 to 2 vascular stenosis of 50%?75%in the main coronary artery or its main beach.The FFR was measured by presser guide wire,and the FFR<0.75 or FFR<0.80 were used as the standard for coronary intervention.The MPI results were evaluated by semi-quantitative methods and eachmyocardial segment was localized to its corresponding comory artery.The efficacy of MPI in diagonosis of myocardial ischemia and the consistency between MPI and FFR were analyzed.The ROC curve of the MPI's total integral(summed differential score,SDS)was plotted.The patients were divided into study group(0.75?FFR?0.8 and no interventional treatment,7 cases)and control group(FFR>0.8,12 cases),follow-up 51 to 82(65.7±8.8)months after surgery.To observe major adverse heart events(MACE,including cardiac death,myocardial infarction,re-reconstruction of blood transport and the incidence of re-angina)and compare event-free survival of two groups.ResultsWhen FFR<0.75,the agreement between MPI and FFR was good(Kappa=0.536),the sensitivity,specificity,accuracy,positive predictive value and negative predictive value of MPI in the diagnosis of myocardial ischemia were 80.0%,87.1%,86.1%,96.4%and 50.0%,respectively.When FFR<0.80,the agreement between MPI and FFR was good(Kappa=0.204),the sensitivity,specificity,accuracy,positive predictive value and negative predictive value of MPI in the diagnosis of myocardial ischemia were 33.3%,85.7%,63.9%,64.3%and 62.5%,respectively.There was no significant difference in the event-free survival rate between the study group and the control group(?2=0.354,P=0.552).ConclusionsWhen FFR<0.75,MPI and FFR has good consistency in the diagnosis of myocardial ischemia.MPI was helpful for assessing the function of coronary artery lesions with 0.75?FFR?0.8 and judging prognosis.
