| Objective To explore the safety and efficiency of patients with incarcerated upper ureter calculus treated by ureteroscopic holmium laser lithotripsy(URHL)and minimally invasive percutaneous nephrolithotomy(MPCNL).Methods 126 cases of incarcerated upper ureteral calculi were treated in this hospita from March 2017 to June2019,including 61 cases of URHL,65 cases of MPCNL,The thotripsy time,intraoperative blood loss,success rate of primary lithotripsy,stone-free rate,postoperative hospital stay,incidence of postoperative complications and other data were analyzed.Results The success rate of group URHL was 88.52%(54/61),the success rate of group MPCNL was100.00%.The stone-free rate one month after operation:URHL was85.25%(52/61),MPCNL was100%(65/65)(P<0.05).In URHL group the thotripsy time(55.54±20.79)min was significantly longer than this(47.77±15.13)min inMPCNL group(P<0.05),but the postoperative hospital stay(2.66±1.12)d shorter than the(4.52±1.51)d in MPCNL group(P<0.05).After the operation,in MPCNL group,8patients developed high fever,inURHL group,8patients had high fever(P>0.05).We found31 patients and 18 patients in MPCNL group and URHL group were pain after operation(P<0.05).Conclusion It has higher successful rate and stone-free rate in patients with upper ureter calculus by MPCNL than those treated by URHL.After 1~2 months 7patients have reoperation in URHL group.The safety and efficency of the former is better that of the latter.MPCNL can be the first choice for the upper ureter calculus. |
