Objective:To explore the value of CpG ODN in chromosome culture of chronic lymphocytic leukemia(CLL),and to explore the cytogenetic characteristics and prognostic value of CLL patients in our hospital.Methods Bone marrow samples from 82 patients with CLL were collected and cultured with PHA,CPG oligonucleotide and interleukin-2(CpG ODN dsp30+IL-2)for 72 hours.The chromosomes were prepared and analyzed by CC.At the same time,D13S25,Rb1,ATM,p53 and csp12 probes were used for FISH detection.The results of chromosome culture and FISH were compared between the two stimulants.Results The success rates of PHA and CpG ODN dsp30+IL-2 were 90.2%(74 cases)and 68.3%(56 cases)respectively,and the detection rates of abnormal karyotypes were 13.5%(10 cases)and 46.4%(26 cases)respectively.The successful rate of karyotype culture in PHA group was significantly higher than that in CpG-ODN DSP30+IL-2group(P=0.01).However,the detection rate of abnormal karyotype in CpG-ODN DSP30+IL-2 group was significantly higher than that in PHA group(P=0.003).The detection rate of FISH abnormal karyotype was 74.4%(61 cases),significantly higher than that of CpG-ODN DSP30+IL-2 group(P=0.000).In CpG-ODN DSP30+IL-2 group,there were 52 kinds of 91 chromosomal abnormalities,46 of which were abnormal in number and 45 in structure.Among them,12(14.6%)were complex abnormalities,3(3.7%)were highly complex abnormalities.The most common chromosomal abnormalities involved were 13q deletion(15.8%),followed by+12(13.4%),and 11q deletion(6.1%).In this study,36 out of 56 CC negative patients were detected by FISH.In 43 cases of FISH detected 13q deletion,CC detected 13 abnormalities in 14 cases of FISH detected+12 CC detected 11 abnormalities in 13 cases of FISH detected 11q deletion,CC detected 5 abnormalities in 5 cases of FISH detected 17p deletion,CC detected 1 abnormality.Conclusion:UsingCpG-ODN DSP30+IL-2 immunostimulation to culture CLL cells can improve the success rate of chromosome karyotype culture and abnormal karyotype detection rate of CLL patients.CpG-ODN DSP30+IL-2 immunostimulation combined with FISH technology can more comprehensively and accurately understand the genetic characteristics of CLL patients,and provide the basis for understanding the disease progress and evaluating the prognosis of CLL patients.
