ObjectiveGroup individualized nursing intervention method based on behavioral change wheel(Behaviour change wheel,BCW)theory for health management of high-risk-risk groups of gestational diabetes,aiming to reduce the incidence of gestational diabetes,cultivate good living habits and promote maternal and child health.MethodsThe GDM high-risk pregnant women who were documented in a third class hospital in Zhengzhou from November 1,2018 to January 30,2019 and planned to give birth in the hospital were selected as the study subjects,according to the chronological order,the subjects included in the study on November 1~December 15 were the control group,the subjects included in the study on December 16~January 30 were the intervention group,the control group was 59,the intervention group was 56,and the intervention group was until the end of the delivery.routine nursing was used in the control group,and the intervention group carried out group individualized nursing intervention on the basis of the control group.before and after the intervention,the self-management questionnaire of maternal health care,the social support scale and the knowledge questionnaire of gestational diabetes mellitus were distributed to carry out a questionnaire survey to compare the incidence of GDM,blood sugar value,weight gain and pregnancy outcome between the two groups.Data are analyzed using SPSS21.0 statistical software.The statistical methods used are descriptive statistical analysis,t test,χ2 test,rank sum test,test level α=0.05.Results1.This study finally included 115 high-risk pregnant women.there was no statistical difference in gestational week,pre-pregnancy BMI、age,highest education,occupation,birth order,place of residence and income between the two groups(P>0.05).the general demographic data of pregnant women in the two groups were comparable.2.GDM incidence and oral glucose tolerance of the two groups were statistically analyzed,and the results showed that the incidence of the control group was 27.1%,the intervention group was 12.5%,the difference was statistically significant;the fasting blood glucose of the control group(4.63±0.46)and the intervention group(4.72±0.50)was not statistically significant(P>0.05);the intervention group 1 h blood glucose(7.29),2 h blood glucose(6.81±1.19)was lower than the control group(8.20±2.14)and 2(7.66±1.09),with statistically significant differences(P<0.05).3.Results of pregnancy outcome in the two groups showed no significant difference in delivery mode,premature infants,gestational hypertension,macrosomia,neonatal hypoglycemia,premature rupture of membranes(P>0.05);weight gain results,weight gain in the two groups after 3 months of intervention was not statistically significant(P>0.05),weight gain in the intervention group after 6 months(13.08±5.61)was lower than that in the control group(16.02±4.92),and the difference was statistically significant(P>0.05).4.Compared with the two groups,the scores of total score and each dimension were statistically significant(P<0.05).5.The comparison of social support level,the difference of total social support score and each dimension score between the two groups was statistically significant(P<0.001).6.The results of knowledge score of gestational diabetes mellitus showed that there was no significant difference in knowledge score between the two groups before intervention(P>0.05);after intervention,the knowledge score of the two groups was statistically significant(P<0.001);before and after intervention,the control group had improved knowledge score,but the difference was not statistically significant(P>0.05),the knowledge score of the intervention group was significantly improved,and the difference was statistically significant(P<0.001).ConclusionsThe group individualized nursing intervention based on BCW theory helps to reduce the incidence GDM high-risk groups of gestational diabetes mellitus,reduce weight gain,help to improve their knowledge level,can improve the self-care management level of pregnant women,and provide more social support.the impact on pregnancy outcome needs to be further explored. |
