BackgroundAtherosclerosis?AS?is the main cause of cerebral infarction and it is also a hot pot for many cerebrovascular disease scholars and experts.Because the carotid artery is an elastic artery with a superficial location,it is a susceptible part of AS,and it is also one of the common affected blood vessels that cause cerebrovascular disease,so it is often used as a research object.Plaque formation is considered a sign of AS.Plaque rupture are still the main cause of death and disability of cerebrovascular diseases.Previous studies have shown that the development of acute ischemic cerebrovascular disease largely depends on the nature of the plaque.Some risk factors,such as Diabetes Mellitus?DM?,smoking and dyslipidemia,will accelerate the progression of arteriosclerosis?CAS?plaque.In the CAS plaque imaging method,the neck ultrasound examination is cheap,fast,contraindication is relatively few,and occupies a place in the clinic,but its diagnostic accuracy depends on the operator's skill and proficiency.High Resolution Magnetic Resonance Imaging?HR-MRI?is a new Imaging technique,which can be used in the differential diagnosis of AS,vasculitis,and carotid dissection.As plaque,not only can accurately assess the nature of plaque,but also has a good diagnostic value for plaque components,especially for high-risk plaque has a good ability to identify,it can predict the risk of cerebrovascular events in vulnerable plaque.ObjectivesPart ? Using traditional ultrasound as a control,to study the consistency between carotid artery HR-MRI in the detection of stenosis and plaque detection in the extracranial carotid artery.Part ? Through HR-MRI examination,analyzes the influence CAS plaque stability the correlation factors.MethodsPart ?A retrospective analysis of 80 in-patients and outpatients with ischemic cerebrovascular disease from January 2018 to December 2019 who underwent both neck ultrasound and HR-MRI was performed,the data of carotid artery stenosis and plaque in80 patients were analyzed by Kappa test.Part ?According to the HR-MRI examination results,74 patients were divided into stable plaque group?34 cases?and unstable plaque group?40 cases?,The collected clinical data to screen the risk factors that affect the stability of CAS plaque byc2 test,two independent samples t test and Logistic regression analysis.ResultsPart ?1.Compared with CAS plaque detection,the consistency of the two methods was general?Kappa=0.47,95%CI=0.190.75?,and the Hypothesis Test U=2.94>U0.05,2=1.96,the difference was statistically significant?P<0.05?.2.There was no significant difference between the two methods in the detection of Carotid Stenosis?Kappa=0.11,95%CI=0.010.20?,and the Hypothesis Test U=1.38<U0.05,2=1.96,the difference no statistically significant?P>0.05?.3.In the detection of unstable plaque,the consistency of the two methods was better?Kappa=0.76,95%CI=0.610.91?,and the Hypothesis Test U=9.5>U0.05,2=1.96,the difference was statistically significant?P<0.05?.Part ?1.The results of single factor analysis showed that age,sex,smoking,drinking,TC,TG,LDL,Hcy,non-HDL and Lipid index were all risk factors of CAS plaque stability,the difference was statistically significant?P<0.05?.2.Age,Hcy,TC and LDL were also independent risk factors of CAS plaque stability,and the difference was statistically significant?P<0.05?.ConclusionsPart ?HR-MRI is superior to ultrasound in detecting vascular stenosis and unstable plaques,It can clearly distinguish plaque components.In contrast,ultrasound is more suitable as a preliminary screening method.Part ?Age,sex,smoking,drinking,dyslipidemia and Hcy were the risk factors of plaque stability,while age,TC,LDL and Hcy were the independent risk factors.
