ObjectiveThe purpose of this study was to explore the role of amplitude integrated electroencephalography(aEEG)in the early assessment of the prognosis of cerebral function in patients with ICU Acute Stroke.Method1.Subjects: A retrospective study of 38 patients with acute stroke who were admitted to to the ICU in our hospital from August 2018 to October 2019.2.Experimental method: The aEEG,Glasgow Coma Score(GCS)were recorded within 72 hours of the onset of the disease, and cerebral performance Category(CPC)score was recorded after 3 months of the onset of the disease.Spearman rank correlation analysis was utilized to test test the correlation between aEEG,GCS,and CPC.Logistic regression analysis was used to evaluate effects of aEEG and GCS classification on the prognosis of brain function. The relevant receiver operating characteristic curve(ROC)was drawn,According to the area under the curve(AUC),sensitivity, specificity and other indicators, the early evaluation effect of aEEG on the prognosis of acute stroke patients in ICU was analyzed.Results1?Among the 38 patients, there were 24 males and 14 females, aged 39 to 89 years, with an average of(64.63±11.55)years.aEEG 10 cases of grade ?,14 cases of grade ?,14 cases of grade ?;10 cases of GCS grade ?,15 cases of grade ?,13 cases of grade ?;11 patients with good brain function and 27 poor patients2?Spearman Rank Rorrelation Analysis: aEEG is positively correlated with GCS(r = 0.730,P <0.01);GCS is positively correlated with CPC(r = 0.586,P <0.01);aEEG is positively correlated with CPC(r = 0.665,P <0.01);3?Logistic Regression Analysis: The gender and age had no statistically significant impact on the prognosis of patients' brain function. In the single factor analysis,aEEG(OR 19.042,95% CI 3.187-113.784),GCS(OR 9.926,95% CI2.272-43.367),both Both P <0.05,suggesting that aEEG and GCS are statistically significant in evaluating patients' brain function prognosis. In multivariate analysis,GCS(OR 4.581,95% CI 0.665-31.531,P> 0.05)was not statistically significant;while aEEG(OR 12.439,95% CI 1.692-91.463,P <0.05)was statistically significant;4?Analysis of the ROC: The AUC of aEEG was(0.897),which was greater than the AUC of GCS(0.871).At the optimal cutoff value of 1.5,aEEG(sensitivity 92.6%,specificity 72.7%)is better than GCS(The sensitivity was 88.9% and the specificity was 63.6%),and the difference was statistically significant(P <0.05).A comparison of the Yoden index found that aEEG(0.653)was higher than GCS(0.525).It is therefore suggested that aEEG is more predictive than GCS in the prognosis of cerebral function in patients with stroke.ConclusionaEEG can be used as an independent factor in the early evaluation of the prognosis of brain function in patients with acute stroke in ICU,and the aEEG predictive ability is higher than that of GCS.
