Objective: This study aims to introduce POCT(Point-of-care Testing)based equipment into primary military medical units.promote the use of infection indicators such as CRP and SAA in primary military medical units.At the same time,the comprehensive analysis of blood routine results can better serve the outpatient and emergency in primary military medical units.Methods: At the beginning of the study,questionnaire survey and field survey were conducted on 74 grassroots troops by stratified sampling method.The content of the survey includes basic information,personnel,instruments equipment and daily work of the unit.Patients with daily respiratory infection who visited the outpatient and emergency department of six grassroots troops from January to December 2018 were selected as the research objects.All patients were infected within 3 days and did not receive any medication before treatment.A total of 734 patients,all male,aged 18-30 years,were randomly selected.Collect 2 ml of venous blood and place in the EDTA anticoagulant tube as preparation.Examine the whole blood routine detection and CRP SAA detection of the samples at the same time.The blood routine was tested by the original hematocrit analyzer of the grassroots troops.CRP SAA was examined by QPAD gold standard digital quantitative analyzer and its supporting reaction cards which produced by Shanghai Upper Bio-tech Pharam co.,LTD.The results were analyzed according to the blood routine,blood routine +CRP or blood routine +CRP+SAA.The positive rates of bacterial and viral infections were obtained and statistically analyzed.Results: According to the survey,the medical inspection work of primary military medical units has a big shortage in personnel,equipment and daily work,which requires faster,more immediate and easier to operate inspection equipment.The prevalence of infectious diseases is the highest in the primary units,and the test items such as blood routine,CRP,SAA and PCT that can be used to judge the condition of infectious diseases are most widely used and in the most urgent need.By using the matched sample chi-square test to analyze the experimental data,the application results of blood routine,CRP and SAA in patients with respiratory infection were obtained.When determining bacterial infection,the diagnostic accuracy of single blood routine index was the lowest.After adding CRP and SAA,the diagnostic accuracy gradually increased.When determining viral infection,there was no statistically significant difference between blood routine test and blood routine test +CRP,indicating that CRP as a test index could not provide an effective basis for determining viral infection.After SAA is added as a determination index,a more accurate diagnosis of viral infection can be made.Conclusion:1.As the situation,task,and the development of technology,the Laboratory medicine work of primary military medical units can't meet the needs of army to improve battle effectiveness.the Laboratory medicine work is limited in equipment,personnel,task conflicts,funds and other aspects.It's need to introduce more quick,easy,practical equipment.2.POCT equipment has obvious advantages over traditional biochemical analyzer in terms of portability,sampling amount,test time and test cost,etc.Moreover,it is easy to operate and easy to learn,and has low requirements on the environment,so it is especially suitable for popularization and application in primary military medical units.3.In the determination of bacterial infection,there were statistically significant differences among the three groups according to the indexes of blood routine,blood routine +CRP and blood routine +CRP+SAA.In the determination of virus infection,when the three indexes of blood routine +CRP+SAA were analyzed,could a better determination result be obtained.4.In the outpatient and emergency work of primary military medical units,blood routine combined with CRP and SAA can improve the diagnostic efficiency and early diagnosis of the pathogen of respiratory disease infection,so as to further improve the rationality of drug use,shorten the course of disease and enhance the combat effectiveness of the troops.
