Background and purpose:Esophageal cancer is one of the world's third major gastrointestinal tumor, the age of onset in more than 40 years old,men are the majority, and its incidence has obvious regional differences[1].The current inspection methods of esophageal carcinoma mainly include esophageal X-ray barium meal and CT?computed tomography?,digestive endoscopy and endoscopic ultrasound, because of the great vessels and heart beating,MRI?magnetic resonance imaging?has been restricted in esophageal examination. In recent years, with the development of MRI technology, some new tests?such as respiratory gating?ECG gating, etc.?and new functional imaging techniques?such as diffusion weighted imaging, magnetic resonance spectroscopy etc.?The MRI in diagn-osis and staging in esophageal carcinoma, and the evaluation of radiotherapy and chemotherapy effect has shown great superiority.DWI is currently the only imaging method that can reflect the movement of water molecules in living tissue. The applic-ation in the parenchymal organs examined more extensively, but seldom used in cavity organ lesions, which is restricted to the single index model DWI[2].This study adopts double exponential model of multi b DWI sequences-intravoxel incoherent motion?IVIM?MR imaging.3.0T magnetic resonance imaging is used to analyse the characteristics of IVIM between the esophageal cancer and normal esophageal, and to evaluate the consistency of quantitative measurement of intra observer and inter obs-erver.Materials and methods:1.Patient data:Collecting patients with esophageal cancer in 21 cases in Affiliated Hospital of Taishan Medical College from December 2014 to December 2015,male 17cases,female 4 cases, age 45-72 years old,the median age was 62.5 years. The patients were confirmed by endoscopy or biopsy confirmed esophageal squamous cell carc-inoma or adenocarcinoma. When the measurement was analyzed, the tumor was reg-arded as a disease group, which was regarded as the normal group and the area beyond the 5cm[6]of the tumor.2.Method:All patients underwent MRI scan,IVIM scan, and without any treatment.Apply GE image workstation and observe the location and morphology of esophageal cancer, and set the region of interest in good levels. Each group is measured Fast ADC value,Slow ADC value,Standard ADC value and Fracti-on of ADC value.Result:There is no difference between normal esophageal tissues and esophageal cancer tissues in the Fast ADC value and Slow ADC value.Compare the Fraction ADC value of normal esophageal tissues?66+8.12?%and esophageal cancer tissues?48.8+8.28?%,P=0,the difference is statistically significant.Compare the Standard ADC value?2.61±0.58?×10-3mm2/s of normal esophageal tissues and esophageal cancer tissues?1.98±0.33?×10-3mm2/s,the difference is statistically significant. The values of ICC between the intra observer and inter observer are greater than 0.75,showing that IVIM-DWI has high measurement reliability in esophagus. Conclusion:The IVIM parameter has a certain difference in the normal esophageal tissues and esophageal cancer tissues, and it can provide a basis for the diagnosis of esophageal cancer. The measurement data of the intra observer and inter observer is good consistency, and is very important for the future of IVIM with further research on esophageal cancer.
