| Purpose Jinan Central Hospital were retrospectively studied 131 patients caused by intrauterine adhesions,through the analysis of the clinical symptoms,menstrual history,history of examination,auxiliary examination,surgical operation,intraoperative score,postoperative measures to prevent caused by intrauterine adhesions,uterine cavity adhesion to explore risk factors caused by intrauterine adhesions and patients caused by intrauterine adhesions separation under hysteroscopy surgery,the effect of intraoperative use of micro scissors,postoperative placed Foley urine tube,using female progesterone artificial periodic uterine recovery after treatment and postoperative pregnancies.Methods in June 2015 to June 2017 in Jinan Central Hospital treatment,131 patients were diagnosed as caused by intrauterine adhesions data were analyzed retrospectively,all patients with preoperative via vagina ultrasound,hysterosalpinography,using tiny scissors separation of adhesion,with reference to China caused by intrauterine adhesions diagnosis classification criteria for grading,postoperative placed Foley urine tube 2-3.5 ml saline to urine tube to form water sac,place 3 to 5 days,use of antibiotics to prevent caused by intrauterine infection postoperatively,vulva nursing study everyday.After blood routine and c-reactive protein were reviewed,and the water capsule was taken out.Postoperative estrogen and progesterone treatment,after the first artificial menstrual clean,2 to 4 days after cycle in the outpatient service line ultrasonic review,postoperative follow-up on a regular basis,understand the patient's menstrual recovery and pregnancies.All data using SPSS22.0 statistical software for processing,count data and measurement data respectively using percentage,mean + /-standard deviation of the format,said the analysis between the measurement data set using single factor analysis of variance,analysis between count data set using X2 test,P < 0.05 think the difference was statistically significant.Results the average age of the 131 patients was 33.56 years old,the oldest of whom was 45 years old,the youngest was 21 years old.Among them,84(64.1%)were younger than 35 years old,and 47(35.9%)were old > 35 years old.131 patients were graded according to the diagnostic grading criteria of Chinese intrauterine adhesion.Slight adhesion 33 cases accounted for 25% :Moderate adhesion of 94 cases accounted for 72%.Severe adhesion 4 cases accounted for 3%.Among the 131 patients,13.7%(18/131)of the patients had no change in their menstrual volume,and 80.2%(105/131)of the patients showed a decrease in menstrual volume,and 6.1%(8/131)showed amenorrhea.Of the 131 patients,14.5%(19/131)had irregular menstruation,and 85.5%(112/131)had menstrual cycles.Of the 131 patients,74.8%(98/131)had abnormal preoperative ultrasound,and 20.6%(27/131)before the operation of the uterine fallopian tube showed abnormal uterine cavity.Among the 131 patients,3.8%(5/131)of patients had a history of intrauterine operation,and 87.0%(114/131)had a history of uterine operation related to pregnancy,and 9.2%(12/131)had a history of non-pregnancy uterine operation.After treatment,the menstrual volume of patients was 9.2%(12/131)without significant changes compared with before treatment.Menstrual volume increased by 90.8%(119/131)After treatment,the rate of pregnancy was 26.7%(35/131).According to the degree of intrauterine adhesion,the pregnancy rate was 27.3%,26.6%,25%,and the abortion was 20%(7/35).Conclusion history of uterine cavity operation has an important influence for caused by intrauterine adhesions,preoperative transvaginal ultrasound can be used as the first option in patients with suspected caused by intrauterine adhesions auxiliary examination project,micro scissors separation caused by intrauterine adhesions under hysteroscopy surgery using Foley urine tube,supplemented by the female progesterone artificial cycle treatment effective in preventing adhesion,again improve menstrual situation,increase the rate of pregnancy. |
