| Objective:To evaluate the value of diffusion weighted imaging in the grading of diffuse gliomas and the mutation status of IDH1 gene.Methods: The ADC values of 90 patients with diffuse glioma confirmed by surgery and pathology were analyzed retrospectively.All patients underwent MRI and diffusion-weighted imaging before surgery,the tumor Parenchyma was measured in 5 regions of interest and averaged.The ADC values of WHO Grade II,WHO grade III and WHO grade IV diffuse gliomas were compared,and the results of immunohistochemistry and gene sequencing were compared.To compare the ADC values between IDH1 gene mutation group and wild-type group and to establish ROC curve to evaluate the diagnostic efficiency of ADC values.At last,we analyzed the ADC values of IDH1 Gene Mutation Group and wild-type group in unenhanced diffuse glioma.Results:There were significant differences in ADC values between WHO II,III and IV grade diffuse gliomas and between WHO II and III grade diffuse gliomas in IDH1 mutant subgroup and wild type subgroup.The sensitivity and specificity of ADC values were 80.00%;60.00%;78.57% and 81.25%,respectively.In addition,compared with wild-type IDH1,the ADC values between the IDH1 mutation group and the unenhanced diffuse glioma group were statistically significant.Conclusion:ADC values can differentiate different grades of diffuse gliomas and may further differentiate the IDH1 mutation status of diffuse gliomas,especially provide quantitative information for the identification of IDH1 gene mutation status of unenhanced diffuse gliomas,to provide imaging basis for clinical treatment plan. |
