Clinical Study Of CT-guided Hookwire Localization Of Pulmonary Nodules

Objective: Lung cancer is currently the malignant tumor with the highest morbidity and mortality.Therefore,early removal of early lung cancer in solitary pulmonary nodules can greatly improve the cure rate of patients.Hookwire localization before SPN resection may improve the success rate and prognosis of patients.This article discusses the advantages,disadvantages,and complications of video-assisted thoracoscopy resection of SPN and Hookwire positioning under CT guidance.Methods: The medical records of 97 patients who underwent preoperative Hookwire positioning and SPN after VATS were included in the exclusion criteria from January 2014 to December 2019 in the Department of Thoracic Surgery,People’s Hospital of Xinjiang Uygur Autonomous Region.The factors for retrospective analysis include the preoperative localization time,the localization success rate,the localization complication and severity,the operation time,the conversion thoracotomy rate,the SPN pathological classification,etc.,to assess the risk factors of localization complications.SPSS 23.0 software was used for data analysis.Chi-square test was used for single factor analysis.Variables for single factor analysis were statistically significant.Further multivariate logistic regression analysis was performed.The difference was statistically significant P value <0.05.Results:The success rate of Hookwire localization was 100%.Among them,23 cases had complications,including micropneumothorax,intrapulmonary hemorrhage,chest pain,mild symptoms,no special treatment,and the patient remitted by himself.The success rate of VATS lung wedge resection surgery was 99%,3 cases were converted to open thoracotomy.Histopathological results after SPN: 51 cases of invasive adenocarcinoma,21 cases of adenocarcinoma in situ,11 cases of atypical adenoma-like hyperplasia,1 case of intrapulmonary lymph node,5 cases of pulmonary tuberculosis,and 12 cases of benign inflammatory lesions.Conclusion: CT-guided Hookwire positioning for VATS surgery for SPN has high diagnostic accuracy,reliable curative effect,few complications,and low severity,and has good clinical promotion value.
