Research On Modern Academic Thought And Clinical Application Of Dymenorrhea Of Fu Qing Zhu's Gynecology

Objective : This paper discusses Fu Shan's understanding of the etiology and pathogenesis of dysmenorrhea,the treatment of dysmenorrhea,and the characteristics of Fu Shan's prescription and medication.By using data mining technology,this paper systematically evaluates the literature on the clinical application of Fuqingzhu's Gynaecology in the treatment of dysmenorrhea prescriptions published in the past 30 year,explores the modern clinical application rules and research status of these prescriptions,finds out their advantages and disadvantages,in order to enrich the research content of Fuqingzhu's Gynaecology,explore new research directions,and provide data support for future research.Methods:Through reading related literature and related books about Fu Qingzhu's treatment of dysmenorrhea,this paper discusses Fu Shan's academic thought of dysmenorrhea treatment.To collect the literature about the clinical application of dysmenorrhea prescription in Fuqingzhu's Gynaecology published in recent 30 years(1990-2019).The database includes: China journal full text database(CNKI);VIP;reading show knowledge database;Wanfang database.According to the inclusion criteria and exclusion criteria,the literature was selected.Extract the relevant information of the included literature,establish a database,standardize the data,use Excel to analyze the metrological characteristics of the literature,analyze the clinical application of each prescription,the drug use rule of each prescription and the change of addition and subtraction;evaluate the quality of the randomized controlled trial,use the modified Jadad scale to score the received literature,and use Revman software to Bias risk assessment,literature heterogeneity test and meta-analysis were performed.Result:1.Fu Shan's treatment of dysmenorrhea mainly focuses on the liver and kidney,which are divided into two ends of deficiency and excess,and mainly on the excess.In the aspect of treatment,we should pay attention to the kidney and liver,strengthen the spleen and treat the liver,spleen and kidney together.We should make good use of the ancient prescriptions,and pay attention to processing.2.Bibliometrics results: a total of 366 documents were downloaded.Through intensive reading,the downloaded documents were eliminated according to the exclusion criteria.The remaining 81 documents were included in this study,including45 clinical trials and 36 clinical experience.There are 74 journal papers,6 degree papers and 1 conference paper.Journal papers come from 50 journals,including 20 core journals and 30 non core journals.34 published in core journals and 40 papers in non core journals.The amount of literature published is increasing slowly.The author unit involves 26 regions,and Jiangsu Province has the most published literature.3.Usage of prescriptions: 81 prescriptions were used for dysmenorrhea 82 times,47 for xuanyutongjing decoction,58 times;2 for jiaweisiwu decoction,2 times;14 for tiaogan decoction,18 times;7 for wenqihuashi decoction,7 times;11 for shunjing decoction,11 times.The quantity of literature published in xuanyutongjing decoction is on the rise,but there is no obvious change in the quantity.The main treatment of Fuqingzhu's Gynaecology dysmenorrhea prescriptions is dysmenorrhea.The syndrome elements are consistent with the efficacy of the prescription.The prescription is mainly used by oral administration of traditional Chinese medicine decoction,and some studies are combined with other methods.4.The use of drugs and the law of addition and subtraction: when xuanyutongjing decoction is used,white mustard and scutellaria are deleted more frequently,and the commonly used new drugs are yanhusuo and chishao,etc.;when jiaweisiwu decoction and tiaogan decoction are used,the original drugs are mainly used,and the loss increasing drugs are less;when wenhuhuashi decoction is used,white fruits are deleted more frequently,and the commonly used new drugs are yanhusuo and cuscuta,etc In the use of shunjing decoction,there are large differences with the original medicine,more new drugs,and various drugs are relatively dispersed.5.Quality evaluation of randomized controlled trials: there are 24 randomized controlled trials,with poor overall quality and low research level.The main problems are: lack of pre-trial and sample size estimation;lack of large sample and multi center clinical research;lack of informed consent and approval of Ethics Committee;inadequate theoretical conditions of trial design;low level of randomization,lack of blind method;dry The standardization level of the pre-treatment is low;the safety is not considered;the measurement methods of outcome indicators are not unified;the follow-up is less.It is better in the balance comparison between groups,research duration,efficacy evaluation,statistical method record and so on.6.In the meta-analysis of randomized controlled trials,the number of literature samples was insufficient,and four literatures were selected for quality evaluation.It is proved that the overall cure rate of Xuanyutongjing decoction is better than that of mifepriston,ibuprofen and other wenstern drugs.Conclusion:Fu Shan's treatment of dysmenorrhea mainly focuses on the liver and kidney,which are divided into two ends of deficiency sample research;the prescriptions reported in clinical report are concentrated in xuanyutongjing decoction,and the related research of other prescriptions is less;can treat many kinds of diseasesthe treatment of disease;When Fu's prescriptions was used in clinic to treat dysmenorrhea,it was mainly used in the original prescription,but there were also some prescriptions with large changes in drug composition and more new drugs;the overall quality of clinical randomized trials was low;there were few literatures in line with meta-analysis.According to the published literature,Fu's prescription for dysmenorrhea is still of great research value in the treatment of gynecological diseases and other diseases.In order to give full play to the clinical application value of the prescription for dysmenorrhea in Fuqingzhu's Gynaecology,we should carry out multi center and large sample research as much as possible,adopt international research standards,and improve the research level.
