Introduction: A cross-sectional study on subjective cognitive decline was conducted to explore the characteristics of TCM syndromes and risk factors of subjective cognitive decline,which provided the evidence for the prevention and treatment of Alzheimer's disease in pre-clinical stage.Method: Between September 2018 and September 2019,Patients admitted to the affiliated hospital of Changchun university of Chinese medicine were selected.The study involved 140 patients.At the end,100 patients completed the test.The scores of SCD9 scale ?5 and AD8 scale ?2 were used as the standard to screen the patients with subjective cognitive decline.Patients' neuropsychological test scores,general information,previous history and information of four diagnoses of traditional Chinese medicine were collected.Clustering analysis was used to explore the characteristics of four diagnoses of traditional Chinese medicine,and multivariate Logistic regression and classification tree were used to explore the risk factors of subjective cognitive decline.Result: Of the 100 subjects,40 were male and 60 were female,with an average age of 67.19±5.5 years.There were 45 patients who met the diagnosis of subjective cognitive decline and 43 patients without subjective cognitive decline.12 patients were suspected to have dementia.According to the results of the information clustering analysis of the four diagnoses of traditional Chinese medicine,patients were mainly divided into four categories: category a is tooth marks on the side of the tongue,fatigue;category b is moss white,chest and short;category c is tongue coating greasy,slippery pulse,fear cold,cold hand and foot;category d is bitter mouth,dry mouth,poor sleep,and constipation.Patients with subjective cognitive decline were named as SCD group,while those without subjective cognitive decline were named as non-SCD group for case-control analysis.There were no statistically significant differences between SCD group and non-SCD group in gender,age,education level,residence style,lifestyle and previous history,etc.In the information feature classification of four diagnoses of traditional Chinese medicine,there were statistically significant differences between SCD group and non-SCD group.Multivariate Logistic regression showed statistically significant differences in syndromes category b(OR=33.04,95%ci: 5.576,310.517),category c(OR=12.31,95%ci: 2.382,80.700),category d(OR=21.03,95%ci: 2.411,487.282)and coronary heart disease(OR=3.20,95%ci: 1.108,10.199)(p<0.05),which were risk factors for SCD.The results of the classification tree showed that the three groups of people with syndromes classified as b,c and d,coronary heart disease in the past history and absence of cerebrovascular disease as risk factors.Conclusion: In this study,it was concluded that the information characteristics of the four diagnoses in SCD were mainly manifested as spleen deficiency,phlegm dampness,dampness and heat,Yang deficiency,phlegm dampness and other syndromes.Spleen deficiency,phlegm-dampness syndrome,dampness-heat syndrome and previous history of coronary heart disease are risk factors for subjective cognitive decline.Compared with multivariate logistic regression,classification tree can better solve the interaction problem between variables,and multivariate analysis can control the interference of confounding factors.
