| [Objective]Developing a questionnaire of home care service needs for advanced cancer patients in Nanjing and testing its reliability and validity,investigating the demand level of home care nursing service for advanced cancer patients in Nanjing and analyzing the main influencing factors,attempting to write home enterostomy care technical specifications draft for advanced cancer patients,preliminary building home care technical specifications for advanced cancer patients in Nanjing.[Methods]1.Questionnaire development:Based on quantitative research and panel discussions,evidence-based screening of home care service needs for advanced cancer patients was conducted from March to April 2019.Based on two rounds of Delphi consultation,the home care service needs of advanced cancer patients in Nanjing were developed and pre-investigated.2.Demand survey:From May to November 2019,980 advanced cancer patients in 15 hospitals in Nanjing were selected by cluster sampling method.The demand level of patients was investigated by the self-made questionnaire on home care service needs for advanced cancer patients in Nanjing,and the reliability and validity of the questionnaire were tested.3.Draft preparation:From December 2019 to February 2020,a drafting team was set up to try to prepare one draft technical specification of home care services for patients advanced cancer.Through the compilation,revision,review,finalization and the compilation of the final meeting,the draft technical specifications of home enterostomy care services for advanced cancer patients in Nanjing(trial version)was determined.[Results]1.Twenty-five articles were included in this study,including IA one articles,IIA eight articles,IIB six articles,IIIA four articles,IIIB three articles,IVA three articles,and a total of 46 home care services for advanced cancer patients were selected.The response coefficient and Kendall's W for the first round of Delphi were 92.0%,0.236,respectively,and for the second round were 100.0%,0.056,respectively.The authority of experts of Delphi surveys was 0.884.The final questionnaire includes 7 dimensions,including educational guidance,auxiliary monitoring,life care,profession-al care,psychological counseling,diet and nutrition,and hospice care,with a total of 43 items.2.The total score of home care service needs of advanced cancer patients in Nanjing was 157.38±18.32,and all the items were 3.66±0.87.Single-factor analysis showed that the scores of home care service needs of advanced cancer patients were statistical differences in age,KPS score,caregivers,marital status,education level,monthly income,and whether home care was feasible(P<0.05).The results of multivariate regression analysis showed that KPS score,monthly income and whether home care was feasible were the main factors.Cronbach's coefficient of total internal consistency of the questionnaire was 0.816,and the retest coefficient was 0.882.The overall consistency of the questionnaire was 0.856 for SCV-I/UA and 0.876 for I-CVI.The average S-CVI/Ave was 0.931.The correlation coefficient r value between each dimension and the total questionnaire was 0.315-0.647.3.The draft preparation team held a panel discussion to write one technical specification of home care service:home enterostomy care,including the content of the technical specification and the operation process,so as to provide a basis for the practice of home care service for patients with advanced cancer in the next step.[Conclusion]This study developed a questionnaire of home care service needs for advanced cancer patients in Nanjing,and then a demand survey was conducted on 980 advanced cancer patients in 15 hospitals in Nanjing.The overall demand of patients was at a medium to high demand level.The main influencing factors were KPS score,monthly income,and whether home care was feasible.At the same time,one home care technology item commonly used by advanced cancer patients were screened,the draft home enterostomy care service technology specifications was organized and wrote(trial version),and the home care service technical specifications for advanced cancer patients in Nanjing was initially constructed,which provided the preliminary basis for the next stage of home care service mode research for advanced cancer patients. |
