| Objective:The clinical manifestations,imaging characteristics and prognosis of basal cell adenoma of parotid gland in wuhu area were retrospectively analyzed.Compared with the two most common benign neoplasms pleomorphic adenoma and adenoma lymphoma in parotid gland and to extract relatively comprehensive,integrated and meaningful diagnostic index.Aimed at improving coincidence rate of preoperative diagnosis of basal cell adenoma of parotid gland,and to provide a reliable basis for the rational decision of surgical methods.Methods:Collect the cases of basal cell adenoma,pleomorphic adenoma and Warthin's tumor of the parotid gland confirmed by surgical pathology during the period from January 2015 to December 2019 in Yijishan hospital of Wannan medical college.Analyzed the clinical manifestations of basal cell adenoma of parotid gland,Including gender,age,duration of disease,smoking history,history of growth and decline,history of multiple tumors,tumor boundary,tumor location,palpation.CT imaging,Including tumor maximum diameter,tumor CT density,tumor CT value,tumor peripheral lymph nodes,whether the tumor has multiple nidus,find out the difference from pleomorphic adenoma and adenoma lymphoma.Collected the prognosis of patients with basal cell adenoma of parotid gland.Including Postoperative salivation,facial paralysis and sweating syndrome.The patients with basal cell adenoma of parotid gland were revisited by telephone whether they had recurrence or malignant changes,and compared with that reported in literature.To identify the difference with pleomorphic adenoma,adenoma lymphoma.Results: A total of 424 patients with basal cell adenoma,pleomorphic adenoma and adenoma of the parotid gland were collected retrospectively,there were 433 lesions.There were 33 patients with basal cell adenoma of parotid gland with 34 lesions.266 cases of parotid pleomorphic adenoma with 266 lesions.Parotid gland lymphoma in 125 patients with 133 lesions.Among them,182 patients had imaging data.It can be found bycomparing the three groups of clinical data: Contrast with pleomorphic adenoma,BCA tends to occur in patients older than 50,It usually occurs in the right parotid gland,and there is no specific location in parotid region.Besides the mass was hard and with no polynesic manifestation.Contrast with Warthin's tumor of the parotid gland,BCA occurs mostly in women,most of the patients had no smoking history and with no history of growth and decline,it was mostly unilateral.Besides the CT value of the parotid gland on plain CT scan of BCA was lower than that of adenoma.All 33 patients with basal cell adenoma of parotid gland were treated with parotid mass and partial glandectomy plus facial nerve anatomy.None of the patients had postoperative salivation and sweating syndrome,three of them had the facial nerve function slight impairment symptom(HB-class II).Because it was a long time,Only 19 patients with basal cell adenoma of parotid gland were associated with postoperative follow-up.None of them had tumor recurrence or malignancy.Three patients with postoperative facial paralysis symptoms recovered within six months,and the symptoms were temporary facial paralysis.Conclusion:Basal cell adenoma of parotid gland is mostly seen in female patients over 50 years old.The tumor is generally hard and prone to occur in the right parotid gland area.Tumors can be seen in the front,back and under the earlobe areas,there is no specific location in parotid region.The activity of tumor is generally good,the boundary is clear,In general,there was no history of multiple tumors,no history of tumor growth and decline,no tumor multifocal feature.Most BCA patients have no smoking history,CT scan value of parotid gland is lower than that of Warthin's tumor and similar to pleomorphic adenoma.Besides the tumor density is generally uniform.The treatment of basal cell adenoma of parotid gland is mainly surgical resection,with better prognosis,lower postoperative complications and recurrence rate. |
