ObjectiveApplying online and offline peer support model to middle-aged and elderly patients with type 2 diabetes in the community,and explore the effect of online and offline peer support to self-management level,quality of life,body mass index(BMI),and fasting blood glucose(FBG)of middle-aged and elderly patients with type 2 diabetes in the community.The objective is to provide reference for future health education and management of middle-aged and elderly patients with type 2 diabetes by community health workers.MethodsThis research is a kind of Quasi-experimental study.Two communities were randomly selected from Tianmashan Street Health Service Center in Hengyang,Hunan Province,and the two communities were randomly assigned to control community or intervention community by coin tossing.From November 2018 to March 2019,Eligible research subjects were recruited from two communities.The control group received routine community care,and the intervention group gave online and offline peer support for 6 months on the basis of conventional community care.Before intervention,at the end of 3 months and at the end of 6 months,the BMI,FBG,diabetes self-management knowledge,attitude,behavior,and quality of life of the two groups of subjects were measured respectively.Related data was entered into SPSS 22.0 software,and ?x ± s or composition ratio was used for statistical description,while two independent sample t test was used,repeated measurement analysis of variance,chi-square test or rank sum test was used for satistical analysis.Results(1)Before the intervention,there was no statistically significant difference between the two groups of patients in the general information,self-management knowledge,attitude,behavior score,quality of life score,body mass index,and fasting blood glucose(P >0.05).(2)Compared with the control group,the patients in the intervention group had higher self-management knowledge and behaviors than the control group at 3 months of intervention(P <0.05);Behavioral and quality of life levels were higher than those in the control group(P <0.05),while FBG were significantly lower than those in the control group at 6 months of intervention(P <0.05).(3)Repeated analysis of variance analysis showed that the self-management knowledge,behavior,quality of life and FBG of the two groups were different between groups(P <0.05);the self-management attitude was only affected by time factors(P <0.05);there was interaction between time and factors in self-management knowledge and behavior,quality of life,FBG and BMI(P<0.05).(4)In control group,there was no significant difference in self-management knowledge,attitude,behavior score,quality of life score,BMI and FBG value at 0,3 and 6 months of intervention(P>0.0167);In the intervention group,the score of self-management knowledge and behavior were higher than before the intervention,and the social relationship score was lower than before the intervention(P<0.0167);At 6 months of intervention,the self-management knowledge and behavior scores were higher than before the intervention,the total score of psychological function,social relationship and quality of life were lower than before the intervention,and the FBG value was lower than before the intervention,the differences were statistically significant(P<0.0167).Conclusions(1)Before the intervention,the patients’ self-management knowledge score was at a medium level,the self-management attitude score was high,the self-management score was at a low level,quality of life was at a medium level,and the body mass index and fasting blood glucose need to be improved.(2)Online and offline peer support can improve the self-management knowledge of middle-aged and elderly type 2 diabetes patients in the community,while this intervention can improve their self-management behavior,quality of life,and fast blood glucose.
