| Objective:Through random grouping and control,to explore the differences in the volume shrinkage rate,PUSH score table,Braden score table,wound bacterial culture and other aspects in the treatment of stage 2 and stage 3 pressure ulcers by external use of the Zhuang medicine Badu Shengji ointment,MEBT/MEBO and b GFG replacement,so as to determine the efficacy of the Zhuang medicine Badu Shengji ointment in the treatment of stage 2 and stage 3 pressure ulcers.Methods:From April 2018 to December 2019,patients with stage 2 and stage 3 pressure ulcers who met the inclusion exclusion criteria and voluntarily participated in the study were selected as the study subjects.Before the implementation of the clinical study,the patients and their families were given health education,and the knowledge of pressure ulcers was popularized,and the patients’families and companions were instructed to take decompression measures.All cases were assessed by the same wound assessment process,that is,"one view,two smell,three touch,four quantity,five take and six record".All cases were selected for routine debridement.After debridement,combined with the stage diagnosis standard,the same stage cases were randomly divided into three groups.The same stage cases were randomly included into the Zhuang medicine Badu Shengji ointment group,MEBT/MEBO group and b GFG group,and each group was separately collected.The treatment was standardized with the Zhuang medicine Badu Shengji ointment,MEBT/meobo and b GFG.The treatment and observation period was 28 days.The shrinkage rate of wound volume was measured before treatment,on the 7th,14th and 28th day,respectively.The PUSH score and Braden score were used to evaluate,and the deep wound samples were collected for bacterial culture.The data were analyzed by SPSS statistics 22.0,and compared before and after the same group,and between groups.Results:In this study,60 patients with stage 2 and 3 pressure sore were included,20 in the study group(Zhuang medicine Badu Shengji ointment group),20 in the control group 1(MEBT/MEBO group),20 in the control group 2(b GFG group),4 patients in the control group 2 quitted,and 56 patients with stage 2 and stage 3 pressure ulcer were included in the final statistical study.(1)Antibacterial Use Density(AUD):there was no significant difference between the three groups(P>0.05).(2)Compared in terms of the following points on the day of 7th、14thand28th:seepage、slouh、granulation tissue、the area and depth of wound,there were significant difference between and among the three groups(P<0.05).Compared the pain score on the day of 7th、14thand 28th,there was no significant difference between the three groups(P>0.05).(3)Braden score:there was no significant difference between the three groups before and after treatment(P>0.05).(4)The volume shrinkage rate of pressure sore:after 28 days of treatment,the volume of pressure ulcer in three groups was compared within the group,the difference is statistically significant(P>0.05).After 28 days of treatment,the pressure sores in the three groups developed towards the direction of gradual healing.In order to determine whether there was any difference between the three groups in the volume shrinkage of pressure ulcers,the volume shrinkage rate of pressure ulcers in the three groups was compared between the groups.The volume shrinkage rate of the three groups was 40.67%,the standard deviation was 24.74%;the volume shrinkage rate of the MEBT/MEBO group was 40.03%,the standard deviation was 31.67%;the volume shrinkage rate of the b GFG group was 40.03%.The small rate was 53.95%,and the standard deviation was21.95%;P>0.05,there was no significant difference in volume shrinkage among the three groups.Through the box chart,we found that the wound surface of each group was gradually reduced.(5)Bacterial culture of wound secretion/pus:there were 56 wounds in the three groups.On the first day,38 wounds were positive for bacteria,accounting for 66%of the total wounds.The most common bacteria in the wounds were Staphylococcus aureus(25.86%),Pseudomonas aeruginosa(12.07%),Escherichia coli(3.45%)and Enterobacter cloacae(3.45%).Three groups of wounds were treated,and there was no significant difference between the three groups(P>0.05).The level of procalcitonin in the three groups was compared within and between the groups(P>0.05),and there was no significant difference among the three groups.Conclusion:Zhuang medicine Badu Shengji ointment,MEBT/MEBO and b GFG can provide nutrients for the growth of granulation tissue and promote the healing of stage 2 and stage 3 pressure ulcers;Zhuang medicine Badu Shengji ointment,MEBT/MEBO and b GFG can improve the local antibacterial ability of the body and control the infection of stage 2 and stage 3 pressure ulcers;however,Zhuang medicine Badu Shengji ointment has no significant effect on improving the local immersion of pressure ulcers.This study confirmed that the treatment of pressure ulcers with the Zhuang medicine Badu Shengji ointment is an economic and effective way.However,due to the limited number of patients included in the experimental study,further research is needed to expand the sample size in the future. |
