Preliminary Construction And Application Of Emergency Triage Schme For Non-traumatic Chest Pain

ObjectiveThe purpose of this study is to establish a set of simple,scientific and practical non-traumatic chest pain emergency triage scheme for emergency nurses,and use this scheme to explore the effects in clinical.Methods1.The formation of the initial scheme : Based on the review of relevant literature,the clinical observation and semi-structured interviews with nurses and patients,we firstly formulated the indicators at alllevels of non-traumatic chest pain emergency triage scheme.Through 2 rounds of expert consultation on indicators at all levels,the scheme formed.2.Preliminary application: The study group adopted the emergency triage scheme for non-traumatic chest pain,and the control group adopted the empirical triage method.A total of 50 patients who complained of chest pain in the emergency department of a certain grade iii first-class hospital in hengyang city from October to November 2018 were selected as the control group.From April to May 2019,50 patients who complained of chest pain were enrolled in the study group for clinical application.The independent sample t-test and chi-square test were used to compare the control group and the test group to verify the clinical effects of the triage scheme.Results1.A non-traumatic chest pain emergency triage scheme was constructed,which included five first-level indicators,chest pain characteristics,systolic blood pressure,finger pulse oxygen,heart rate,and electrocardiogram.Each of the first-level indicators was set with specific ?,?,and ?level indicators.The positive coefficients of the two rounds of expert consultations were 100% and 96.1%,the expert authority coefficients were 0.78 and 0.78,and the degree of coordination was 0.444 and 0.489(P<0.01).The coefficient of variation of the importance of the first-level indicators after the two rounds of consultation was 0.00 ~ 0.22.2.A historical control study was used to initially establish the non-traumatic chest pain emergency triage scheme.The results showed that the study group's first ECG time was(4.24 ± 2.30)min;the time from triage to troponin report was(17.78 ±7.62)min;The waiting time was(3.22±1.49)min;the accuracy of triage was 90%;The average satisfaction of each diagnosis scheme was higher than that of the control group.The study group's first ECG time,time from triage to troponin report,triage accuracy,and nurse satisfaction were all higher than those in the control group,and the differences between the two groups were statistically significant(P<0.05).Conclusions1.Thenon-traumatic chest pain emergency triage scheme sdeveloped in this study have high authority and good coordination.2.The use of non-traumatic chest pain emergency triage scheme could shorten the first ECG time,the time from triage to troponin report,the waiting time,and improve triage accuracy and emergency nurse satisfaction.
