Gliomas are the most common primary neuroepithelial tumors in the central nervous system,accounting for 33.3-58.6%of intracranial tumors.Imaging examination,including CT and MRI,is an important tool for clinical diagnosis of gliomas and tumor staging.In particular,MRI has the advantages of high resolution of soft tissue,no radiation,high repeatability,high accuracy,short scanning time,various scanning procedures and reconstruction methods.It plays an increasingly important role in the early diagnosis and clinical follow-up of gliomas.3D-ASL is not only radiation-free,easy to operate and highly repetitive,but also suitable for patients with severe renal insufficiency.More importantly,3D-ASL can accurately reflect the degree of neovascularization in neoplastic lesions and evaluate the hemodynamics of brain tumors.Not only that,it also has the distinguishing significance to the tumor relapse after treatment and the radiotherapy injury.In addition,some specific molecular markers can be detected for the occurrence,development and early malignancy of tumors,and their expression is often closely related to the prognosis and clinical treatment of tumors.Ki-67 is an important indicator of cell proliferation activity and is closely related to tumor proliferation,stage and prognosis.Ki-67 was significantly different between the normal brain tissue and the high and low grade of glioma,and was correlated with the high and low grade of glioma.GFAP is an important marker protein of astrocytogenic tumor,which supports and nourishes neurons and participates in many biological functions of astrocytes,including cell proliferation,division,maintenance of physiological function of blood-brain barrier,autophagy,maintenance of neurotransmitter balance,etc.Therefore,in this study,preoperative 3D-ASL imaging was used to quantitatively detect the cerebral blood flow of patients with glioma of different grades.Tumor tissues were obtained by surgical resection and expression levels of Ki-67 and GFAP were detected.Correlation between imaging and immunological indicators was analyzed to provide reference for accurate and targeted treatment of glioma and follow-up after treatment.Objective:Preoperative 3D-ASL imaging was used to quantitatively detect the cerebral blood flow of patients with glioma of different grades.The tumor tissues were obtained by surgical resection and the expression levels of Ki-67 and GFAP were detected.The correlation between imaging and immunological indicators was analyzed to provide reference for the accurate and targeted treatment of glioma and the follow-up after treatment.Method:A total of 62 patients with glioma confirmed by operation and pathology were randomly selected from June 2018 to October 2019,including 35 patients with low-level(?-?)and 27 patients with high-level(?-?).Before operation,the maximum tumor blood flow(TBFmax)was obtained by 3D-ASL perfusion imaging and dynamic enhancement,and the ratio to blood flow of the contralateral hemisphere,gray matter and white matter(rCBF)was calculated,immunohistochemistry and Western blot were used to detect the expressions of Ki-67 and GFAP in tumor and adjacent normal tissues.Results:The TBFmax,hemispheric rCBF,gray matter rCBF and white matter rCBF values in low-level group were significantly lower than those in high-level group[(1.23±0.34)VS.(1.89±0.56),t=5.006,P=0.004;(1.12±0.41)VS.(1.45±0.58),t=4.785,P=0.013;(1.26±0.48)VS.(1.58±0.69),t=4.659,P=0.016;(1.33±0.39)VS.(1.75±0.68),t=4.923,P=0.009].The positive expression rate and quantitative expression level of Ki-67 in tumor tissue of low-level group were significantly lower than those of high-level group,while these of GFAP in low-level group were higher than these in high-level group(P<0.05).It was found by Pearson test that TBFmax,hemispheric rCBF,gray matter rCBF and white matter rCBF were positively correlated with the quantitative expression level of Ki-67 in tumor tissue,but negatively correlated with the quantitative expression level of GFAP(P<0.05).Conclusion:By comparing the cerebral blood flow and the expression of specific molecular markers in patients with glioma of different grades,the quantitative detection of cerebral blood flow by preoperative 3D-ASL imaging can predict the expression levels of Ki-67 and GFAP in glioma,which provides important reference value for the prediction of tumor prognosis,molecular targeted therapy and post-treatment follow-up.
