Regularity Of Mood Episodes In 649 Cases Of Patients With Bipolar Disorder

Objective:To understand the regularity of the different mood episodes(such as depressive episode,manic episode or mixed episode)in patients with bipolar disorder(BD)during multiple admissions for further acquaintance of the clinical character of the paroxysm of BD.Methods:A total of 649 inpatients diagnosed as BD in accordance of ICD-10 at the last discharge were studied.Use the customized collection table to collect the data of mood phases at multiple admissions.Then analyze the regularity of different phases during the progress and calculate the phase consistency rate,phase conversion rate,delayed diagnosis rate,delay-time of diagnosis and misdiagnosis rate at every single time of admissions.SPSS software was used for statistical analysis.Results:Among the 649 cases,there were 355(54.7%)in group bipolar-mania,230(35.4%)in group bipolar-depression group and 64(9.9%)in group bipolar-mixing.1.The phase consistency rate:384 patients were admitted with the same clinical phase each time,and the phase consistency rate was 59.2%.Among them,the rates in the group bipolar-mania,bipolar-depression and bipolar-mixing were 71.5%(254 cases),43.9%(101 cases)and 45.3%(29 cases),respectively.It was shown that there were statistical differences between the groups(?~2=49.765,P=0.001),and the group bipolar-mania was significantly higher than the other two groups(P<0.05).2.The phase conversion rate:195 patients had different phases of mania,depression or mixed episodes during previous hospitalization,that is,the phase conversion rate was 30.0%(195 cases).The phase conversion rates in group bipolar-mania,bipolar-depression and bipolar-mixing were18.5%(66 cases),47.8%(110 cases)and 29.7%(29 cases),respectively,and also there were statistical differences between the groups(?~2=56.758,P=0.001).The phase conversion rate was the highest in the group bipolar-depression,the second in the bipolar mixed group and the lowest in the bipolar mania group.3.The delayed diagnosis rate and delay-time:The delayed diagnosis rate was 14.0%(91 cases)on the whole,while 7.0%(25 cases)in the group bipolar-mania,23.5%(54 cases)in the group bipolar-depression and 18.8%(12 cases)in the group bipolar-mixing.There was a statistical difference between the three groups(?~2=32.592,P=0.001).The rate of group bipolar-mania was significantly lower than them of the other two groups,the differences were statistically significant(P<0.05).The average delay-time of diagnosis was 2.77±1.86(0.03-8.52)years,and there was no statistical difference between the groups(F=0.086,P=0.917).4.Previous diagnosis rate of non-mood disorder:70 patients were diagnosed as non-mood disorder when they were hospitalized for the first time,that is,the previous diagnosis rate of non-mood disorder was 10.8%.The rates of bipolar mania,bipolar depression and bipolar mixing were9.8%(35 cases),8.3%(19 cases)and 25.0%(16 cases),respectively.There was a statistical difference between the three groups(?~2=32.592,P=0.001).The rate of group bipolar-mixing was significantly higher than them of the other two groups,the differences were statistically significant(P<0.05).Conclusion:There were certain patterns of mood episodes in patients with BD during each admission in our research hospital,and 59.2%(384 cases)of the patients were admitted in the same clinical phase,of which bipolar mania was more common.Others had phase conversion(30.0%,195 cases)and/or delayed diagnosis(14.0%,91 cases)and/or misdiagnosis(10.8%,70 cases).Group bipolar-depression was more prone to delayed diagnosis and group bipolar-mixing was more likely to the diagnosis of previous non-mood disorders.
