Objective:To take the critical patients in emergency department of a third-class hospital in Jingzhou as the object,introduce the concept of list based on evidence,summarize the key information and leak-prone links into a unified standard,implement the key points and key contents in writing,form an efficient and simple transfer list of emergency critical patients and apply it to clinic,shorten the transfer time of critical patients,improve the efficiency of medical staff,reduce the occurrence of adverse events in transit,and ensure the safety of patients.Methods:(1)to consult the relevant literature of the transportation of critical patients at home and abroad and the advice of experts in the related disciplines of critical illness,and to design the transfer list of emergency critical patients in accordance with the actual situation of our hospital by themselves combined with the opinions of the first-line clinical medical staff in the emergency department of a third Class A Hospital in Jingzhou City.(2)With the approval of the Hospital Ethics Committee,160 patients with acute and critical illness requiring hospital transit from December 2018 to September 2019 were selected by the convenience sampling method.Referring to the "Guidelines for Classification of Emergency Patients",according to the actual situation of the hospital emergency department,the inclusion criteria were:critical patients in need of transport,under the condition of meeting the MEWS score>5,or the following conditions:1.Shock;2.Severe fractures,multiple injuries;3.Cardiovascular and cerebrovascular accidents,angina,severe chest pain;4.Organ failure;5.Airway obstruction,6.Massive hemorrhage,7.Acute poisoning;8.Surgical acute abdomen;exclusion criteria:1.Critical patients with uncontrolled mental symptoms;2.Critical patients with accidental death on transit.80 critical patients transferred from emergency department from?eptembe 2018 to march 2019 were used as control group and 80 critical patients transferred from emergency department from june 2019 to III eptember 2019 as experimental group.Compare the required transit time and the incidence of adverse events between the two groups of critical patients during transit.(3)Using EpiData software to establish database,using SPSS 19.0 software to process data,using descriptive statistics,t test,?2 test and so on to carry on statistical analysis.Results:(1)Comparison of transport time between the experimental group and the control group:1.The transport time(7.050±2.385,9.200±5.165)between the emergency department and the surgical department was significantly lower than that of the control group(10.375±5.314,17.025±14.907).2.The time between emergency department and catheter room(2.000±2.445)in the experimental group was lower than that in the control group(7.750±5.737),but the difference was not statistically significant.(2)Comparison of the incidence of transport adverse events between the experimental group and the control group:1.The incidence of total adverse events in the experimental group(13.75%)was significantly lower than that in the control group(38.75%),and the difference was statistically significant;2.Comparison of the incidence of,the incidence of severe adverse events group and the control group:the incidence of severe adverse events in the experimental group(11.25%)was significantly lower than that in the control group(27.5%);3.Comparison of the incidence of high-risk events in transport between critical patients in the experimental group and the control group:the incidence of high-risk events in the experimental group(2.5%)was significantly lower than that in the control group(11.25%).The difference was statistically significant.Conclusion:based on evidence-based,applying the concept of the list to the"emergency critical patient transport handover list" formed in the transport of critical patients can effectively shorten the transit time of critical patients,improve the working efficiency of medical staff,reduce the incidence of adverse events in transit,and ensure the safety of transportation of critical patients.
