BackgroundLeukemia is the most common malignant tumor in children.Its treatment is complicated,the mortality rate and the cost of treatment are high,which causes a heavy economic burden on the sick families.If there is no proper medical security and compensation mechanism,it is easy to cause poverty due to illness.The Z project was jointly initiated by a number of charitable foundations.The main measure is to increase the cost of the basic medical insurance for children with leukemia in the pilot area to 90%,and the purpose is to eliminate the problem of poverty caused by childhood leukemia.Calculating the current economic burden of disease in children with leukemia,analyzing and comparing the compensation effect of charitable medical assistance projects for the assisted families is of great significance for the government to scientifically and rationally integrate and distribute the assistance resources for serious diseases,and to explore a high proportion of medical insurance reimbursement models for serious diseases.PurposesBased on the implementation of the Z project to increase the reimbursement rate of children's leukemia in Heyuan City,the increase in the reimbursement ratio of medical insurance is regarded as a compensation factor for leukemia medical expenses.The policy of leukemia reimbursement ratio studies the compensation effect of families with leukemia children,provides reference for other charitable organizations to participate in the medical assistance model,and provides a certain basis for the improvement and adjustment of the national major medical insurance reimbursement policy.MethodsTaking the family members of 64 leukemia children who were rescued by Project Z from August 2017 to July 2019 in Heyuan City,Guangdong Province as the research object,collect their social security data,supplement the reimbursement data and conduct a questionnaire survey.Among them,17 typical cases were selected for qualitative interviews to obtain evaluation data.First,calculate the economic burden of leukemia children's disease,analyze the difference of direct medical expenses in the three cases of basic medical insurance,critical illness insurance,and Z project supplemental reimbursement.A multi-dimensional analysis of the main indicators of the compensation effect of the Z project supplementary reimbursement policy for families with leukemia children.Results(1)The economic burden of family diseases of children with leukemia is heavy.The total economic burden of per capita disease of children is 305,600 yuan;the direct medical burden is 231,300 yuan,accounting for the largest proportion of the direct economic burden(2)After being reimbursed by basic medical insurance and critical illness insurance,the medical expense reimbursement rate of the child is 51.84%.After supplementary reimbursement through Project Z,the actual reimbursement ratio reached 72.75%,the proportion of self-pay decreased significantly,and the economic burden of disease was significantly reduced.(3)After supplementary reimbursement by the Z project,the incidence of poverty caused by illness in the family of the child decreased from 67.18%after the major illness insurance reimbursement to 45.31%;Catastrophic health expenditure rate decreased from 56.25%after major illness insurance reimbursement to 20.31%;Indicators such as the breadth and depth of poverty caused by illness,the average disparity and relative disparity in catastrophic health expenditures have also declined to varying degrees.(4)Through qualitative research,Z project supplementary reimbursement eased the economic burden and poverty of children's family disease,and also plays an important role in enhancing the treatment confidence of family members and alleviating family relationshipsConclusions and SuggestionsThis study found that the economic burden of the family disease of children with leukemia is heavy,and the basic medical insurance and the critical illness medical insurance have a limited role in alleviating the economic burden of family disease of children with leukemia.Atastrophic health expenditures and other indicators have improved to varying degrees.However,the Z project supplementary reimbursement policy is still in the stage of development and improvement,and there are still some shortcomings such as the inability to reimburse drugs outside the medical insurance catalogue,limited coverage of areas and diseases,and other areas still need to be further improved.Suggestions:(1)Reasonably control medical expenses and reduce disease economic burden;(2)Give full play to the joint guarantee effect of charitable medical assistance and government medical insurance system;(3)Reasonably compensate for off-catalog expenses,especially self-paying medicine expenses;(4)Improve the charity medical assistance project model and create a good development environment..
