Objective:To obtain evidence-based medical evidence,we conducted a Meta-analysis of the correlation between Helicobacter pylori(Hp)infection and Carotid intima-media Thcikness(CIMT).A retrospective study was conducted on part of the physical examination population in the health management center of Binzhou medical university hospital,to explore the correlation between Hp infection and CIMT.To determine whether Hp infection is a new independent risk factor affecting the development of atherosclerosis,and to provide data support and scientific basis for the prevention and treatment of carotid atherosclerosis.Method:In the Meta-analysis of the correlation between Hp and CIMT,lit eratures published from January 1,2000 to December 31,2018 were searched.Using Q test and I2 statistic conduct the heterogeneity analysis of the nine art icles?According to the results,select the randomized effect model or a fixed effect model,calculate combined effect value WMD(weighted mean difference s),95%confidence interval(95%CI)and P values.if the results have considerabl e heterogeneity,then conduct subgroup analysis,sensitivity analysis,and finally adopt Egger's inspection analysis of publication bias.Retrospective analysis of the physical examination data of some people who underwent physical examination from March 2015 to July 2015 in the health management center of Binzhou Medical University Hospital was conducted,which mainly included gender,age,family history of hypertension or coronary heart disease or stroke,history of smoking and drinking alcohol,salt intake,BMI,blood pressure,blood lipid,fasting blood-glucose,Hcy,CRP,the test results of Hp infction and abdominal ultrasound examination.131 subjects consisted of thickening CIMT group of 60 cases and normal group of 71 cases.All cases were examined with carotid sonography.Making form to collect data and referring to previous related literature about Hp infection and CIMT correlation research,univariate analysis and multivariate conditional logistic regression analysis were used to screen the risk factors and protective factors of the change of CIMT.Results:A total of 9 Chinese and English literatures were used in this meta-analysis.A total of 1961 cases were included in the study,consisting of 1033 cases of helicobacter pylori positive and 928 cases of helicobacter pylori negative.Meta-analysis results showed that the CIMT of helicobacter pylori positive patients was higher than that of helicobacter pylori negative patients,and the difference was statistically significant[combined effect value(WMD)=0.06mm,95%confidence interval(95%CI):0.03?0.09,p<0.05].The subjects were divided into the high-risk group and the general group,and the results showed that the CIMT of the hp-positive group was higher than that of the hp-negative group,and the difference was statistically significant[WMD=0.06mm,95%ci:0.03?0.09,p<0.05].In the Logistic regression analysis between Hp infecttion and CIMT,univariate analysis was used to select the influencing factors for the thickening of CIMT.The results indicated that the difference in age,gender,BMI,smoking history,blood pressure,Hp infection,FBG,TG,LDL,HDL,Hcy and CRP between the carotid atherosclerosis group and the normal group was statistically significant(P<0.05).The results of multivariate Logistic regression showed that the OR values of smoking history,systolic blood pressure,FBG,Hp infection and LDL in this study were all>1,P<0.05;HDL OR<1,P<0.05,suggesting that they were related risk factors and protective factors affecting CIMT.Conclusion:Hp infection is associated with CIMT thickening,and Hp infection may be an independent risk factor for carotid atherosclerosis.
