Objective:Application of centricity,Degree Centrality,DC),low frequency oscillation amplitude(amplitude of low frequency fluctuation,ALFF)and local consistency(regional homogeneity,ReHo)three methods to observe the cerebral apoplexy patients with dual task before and after the training areas of the brain activation,and the brain regions activated function connection and elaborates the physiological function of therapy for the dual task of promotion to provide effective neural physiological basis.Methods:32 patients with subacute cerebral apoplexy who were included in the department of rehabilitation medicine of the second affiliated hospital of kunming medical university and met the 2015 version of the diagnostic criteria for stroke were trained for 3 weeks,and 16 patients were randomly assigned to the experimental group for dual task rehabilitation training and routine rehabilitation training.In the control group,16 patients underwent routine rehabilitation training and corresponding clinical function evaluation.Patients who met the inclusion criteria were selected as the fMRI observation group to receive DT treatment and magnetic resonance imaging.MOCA cognitive assessment scale,mini-best balance scale and Barthel index(BI)score were used for clinical evaluation.Rs-state functional MRI(rs-fmri)scans were performed before and after dual-task rehabilitation in 10 stroke patients in the fMRI observation group.Rs-fmri data were used to analyze the changes of brain functional connections before and after training by DC,ALFF and ReHo values.Results:1.After 3 weeks of training,the balance function(mini-best balance scale)and cognitive function(MOCA cognitive assessment scale)of both groups had significant changes(p<0.05).However,by comparing the changes before and after training,the dual-task training was better than the conventional rehabilitation training in improving the balance function and cognitive function of patients(p<0.001).2.The fMRI results of 10 subjects in the fMRI observation group showed that the caudate nucleus and precuneus DC value increased after training,while the difference between ALFF value and ReHO value was not significant.Conclusions:1.Both dual task training and single task training can effectively improve the balance and cognitive functions of stroke patients,but the effect of dual task training is better than that of conventional rehabilitation.2.Changes in brain functional connections were observed at the voxel level,and the caudate and precuneus were closely related to the neural recovery mechanism in the brain after dual task training.
