Objective:To investigate the correlation between prethrombotic state and systemic inflammatory response by studying the levels and relationships among IL-6,hs-CRP and prothrombin time(PT),D-Dimer(D-D),fibrinogen(FIB),and hematocrit(HCT),activated partial thrombopastin time(APTT)in serum of COPD patients,the correlation between prethrombotic state and systemic inflammatory response was speculated,and the mechanism of prethrombotic state in COPD patients was discussed.The correlation between inflammatory index,prethrombotic state index and lung function was studied.Methods:A total of 100 patients with AECOPD who were hospitalized in the second affILiated hospital of Kunming Medical University from January 2018 to June 2019 were selected,including 82 males and 14 females,with an average age of 73.21±7.26 years.There were 69 patients with COPD who were in remission and stable after treatment,including 59 males and 10 females,with an average age of 72.81±8.4 years.FEV1,FVC,FEV1%pred and FEV1/FVC were detected,which were divided into mILd(FEV1%pred>80%),moderate(50%?FEV1%pred<80%),severe group(30%<FEV1%pred<50%),and extremely severe group(FEV1%pred<30%)according to the GOLD classification standard.During the same period,54 patients with no infection and normal lung function examination in our hospital were selected as the control group,including 49 males and 5 females,with an average age of(72.48±8.06)years.The name,sex,age,weight,height,body mass index(BMI),smoking status and other medical history of all participants were recorded.Serum levels of IL-6,hs-CRP,APTT,PT,D-D,FIB and HCT in each group were detected,and changes in serum inflammatory factors and coagulation indicators in COPD patients and their correlation were analyzed.To analyze the correlation between inflammatory index and pulmonary ventILation function.Results:1.Analysis of the basic data of each group,there was no significant difference in sex composition,age and BMI between the three groups(P>0.05).2.The serum levels of IL-6 and hs-CRP in AECOPD group were significantly higher than those in SCOPD group and control group(P<0.05);The levels of IL-6 and hs CRP in the SCOPD group were higher than those in the control group(P<0.05).3.The levels of D-D and FIB in AECOPD group were significantly higher than those in SCOPD group and control group(P<0.05),but there was no significant difference between SCOPD group and control group(P>0.05).The levels of APTT in AECOPD group was significantly shorter than that of control group(P<0.05),and there was no significant difference between AECOPD group and SCOPD group(P<0.05).The serum HCT of AECOPD patients was significantly higher than that of the control group(P<0.05),and there was no significant difference between the AECOPD group and the SCOPD group and the control group(P<0.05).There was no statisticaldifference in PT between the three groups(P<0.05).4.IL-6 was significantly positively correlated with D-D and FIB(P<0.05)in AECOPD group,but not with HCT(P>0.05);hs-CRP was positively correlated with D-D,FIB and HCT(P<0.05).IL-6 and hs-CRP were significantly positively correlatedwith D-D,FIB and HCT in SCOPD group(P<0.05).5.In AECOPD group,IL-6 and hs-CRP were negatively correlated with FEV1%pred and FEV1/FVC,and the difference was statistically significant(P<0.05),but not with FEV1(P>0.05);in COPD stable group,IL-6 and hs-CRP were negatively correlated with FEV1%pred,FEV1,FEV1/FVC,and the difference was significant(P<0.05).Conclusions:1.The expression of IL-6 and hs-CRP was negatively correlated with pulmonary ventilation function in COPD and played a role in chronic airway response in COPD;IL-6 and hs-CRP were correlated with the degree of airflow limitation in COPD.2.COPD patients have a prethrombotic state with a risk of thrombosis,which is more pronounced in acute exacerbations,and the pre-thrombotic state is associated with systemic inflammatory response,which can promote thrombotic risk.COPD is a chronic inflammatory disease characterized by a hypercoagulable state.3.The level of serum inflammatory markers in acute exacerbation of COPD was significantly increased,further verifying that chronic obstructive pulmonary disease is a systemic inflammatory disease,and the level of inflammatory markers in stable COPD was also increased,indicating that COPD has persistent systemic low-grade inflammation.
