ObjectiveThrough the summary and analysis of the clinical data of 500 inpatients with herpes zoster,to explore the related characteristics of herpes zoster,to analyze the distribution characteristics of herpes zoster ganglion,to study its distribution regularity,and to explore the influencing factors of post-herpetic neuralgia,so as to provide reference for clinical diagnosis and treatment.MethodsThe clinical data of patients with herpes zoster hospitalized in the Department of Dermatology,Henan people’s Hospital from September 2017 to September 2019 were selected and collected,including the patient’s name,age,sex,inducement,season of onset,sequence of pain and rash,classification of herpes zoster,distribution of skin lesions,pain score during hospitalization,underlying diseases,occurrence of post-herpetic neuralgia and other complications,blood calcium value and so on.SPSS24 software was used for statistical analysis.The counting data were analyzed by percentage(%),and the measurement data were expressed by mean ±standard deviation.Chi-square test and univariate analysis combined with Logistic regression analysis were used to explore the influencing factors of post-herpetic neuralgia.Results500 hospitalized patients with herpes zoster were screened successfully,of which:1.Patients are mainly over 50 years old,accounting for 70.60%;female patients account for 51.40%,male patients account for 48.60%,women are slightly more than men,men are more than women in people under 50 years old,and women are more than men in people over 50 years old.The most common cause of disease is fatigue,accounting for 28.20%.Herpes zoster can occur all year round,but it occurs frequently in autumn and summer,accounting for 57.60%.The proportion of skin rash after pain was the largest(59.20%),followed by pain(15.40%),pain and rash(25.40%).Among the types,vulgaris,bullous type,gangrenous type,hemorrhagic type,sudden contusion type and disseminated type accounted for 77.20%,9.80%,3.40%,3.20%,2.60% and 3.80%,respectively.There was no significant difference between the left and right sides of the disease site,the left side accounted for 49.60%,the right side accounted for 48.60%;the incidence rates of intercostal nerve region,trigeminal nerve region,cervical nerve region and lumbosacral nerve region were 44.40%,27.00%,14.80% and 11.60%,respectively.The intercostal nerve region is the most,followed by the trigeminal nerve region.Moderate pain(4-6 points)was the most common pain score during hospitalization,accounting for 72.80%.There was no significant difference in pain scores among patients of different ages;55.20% of patients were associated with underlying diseases;and 17.45% of patients had decreased serum calcium levels.2.The largest number of patients were involved in the two ganglia,the highest incidence of skin lesions was in T1,T5 and T4 S5,and the number of trigeminal nerve involvement was the most in a single ganglion(12.50%).There was no significant difference in the left and right distribution of skin lesions,the involved nerve segments and the number of involved ganglia among patients of different ages(P> 0.05).There was no significant difference in the left and right distribution of skin lesions,the number of involved nerve segments and the number of involved ganglia in patients of different genders(P> 0.05),but there were significant differences in the scores of involved nerve segments and pain degree(P =0.003 < 0.05).3.15.00% of the patients with herpes zoster developed post-herpetic neuralgia,and age,number of involved ganglia,pain degree score,disease classification and blood calcium level were all related to the occurrence of post-herpetic neuralgia in univariate chi-square test.In further multivariate regression analysis,age,pain score at hospitalization and disease classification were independent risk factors for post-herpetic neuralgia.Conclusions1.The intercostal nerve area of herpes zoster is the most common,the trigeminal nerve is the most frequently involved in a single ganglion,and post-herpetic neuralgia is the most common complication of herpes zoster.2.Age,pain score during hospitalization and disease classification were independent influencing factors of postherpetic neuralgia.
