The Alienation Of The Medical Social Work Department:an Analysis Based On The Efficiency Mechanism And The Legality Mechanism

The state pays more and more attention to the development of medical social work,and incorporates the setting of medical social work posts into an improved indicator system for medical services.The scholars of various academic circles have gradually developed their research on medical social work,and have made great achievements in both theoretical and practical fields.However,the author finds that when scholars study medical social work,the organization of medical social work is always ignored,and even the number of times mentioned in national policies is relatively small.At the same time,as a department of the hospital,the Medical Social Work Department is still affected by the hospital’s logic of action.The setting of the Medical Social Work Department of H Hospital is not to provide medical and social work services,but to cope with improving the evaluation of medical services and obtaining medical expenses in disguise.In the course of setting up the medical social work department,the hospital "alienated" it.Although it is called the"Medical and Social Work Department",it cannot fulfill the goal set by the policy-"Relieve doctor-patient contradictions and provide convenient services for patients".And this "alienation" condition has greatly hindered the healthy operation of the medical social work department,which should have been paid attention to.But unfortunately,the academic research on the phenomenon of "alienation" of medical social work department is almost zero.Therefore,based on my two-month internship,the author explores the "alienation" of the Medical Social Work Department of H Hospital through participatory observation and semi-structured interviews.As an intern of medical social work in H hospital,the author makes a participatory observation on the interaction between medical social work department and hospital,between medical social work department and functional department and business department in hospital,and between medical social workers and medical staff,and conducted a semi-structured interview with the director and intern of medical social work department,medical staff of decentralized department and patients.It is found that the reason of "alienation" of medical social work department is largely influenced by the mechanism of legitimacy and efficiency and its relationship.H hospital set up the medical social work department based on the principle of legality,but based on the principle of efficiency,the hospital pursues the maximization of benefits.As a department of the hospital,the medical social work department,because of the limitation of hospital resources and the pursuit of legitimacy status,has to bear the oppression of the hospital and survive in the gap.Because can not rely on their own strength to correct their own direction of development,so the medical social work department can only be "alienated" state.To promote health care Good operation,based on the perspective of legitimacy mechanism and efficiency mechanism,the author tries to put forward the relevant solutions:play the protective role of legitimacy mechanism,avoid the restriction of efficiency mechanism,prevent the vicious transformation of legitimacy mechanism power.
