Objective:To study and explore the ideal administration mode and dose of TXA in simultaneous bilateral total knee arthroplastyMethods:From January 2015 to December 2018,92 patients undergoing bilateral total knee arthroplasty during the same period were divided into double dose group(group A)and multiple dose group(group B).45 patients in group A received intravenous drip of TXA 15mg/kg 15 minutes before the upper tourniquet,local injection of TXA 15mg/kg after the closure of the joint capsule on the other side(dominant side accounted for 46.6%);47 patients in group B received intravenous drip of TXA 15mg/kg 15 minutes before the upper tourniquet,and 15 mg/kg after the closure of the joint capsule on the other side and intravenous drip of TXA 15mg/kg after operation(dominant side accounted for 51.1%).Record gender,sex ratio,body mass index,preoperative blood volume,preoperative hemoglobin level,circumference of legs and thighs,ROM、HSS and VAS scores were recorded.Total blood loss,blood transfusion,days of hospital stay、occurrence of symptomatic venous thromboembolism(VTE),circumference of legs and thighs and ROM、HSS and VAS at 6 months after operation were compared between the two groupsResults:Postoperative dominant blood loss,latent blood loss and the maximum decrease of hemoglobin in group B were better than those in group A(P<0.05),and blood transfusion、ROM、HSS、VAS in group B was significantly better than that in group AConclusion:Multiple application of local use of TXA during simultaneous bilateral TKA can effectively reduce the amount of bleeding without increasing the risk of VTE...
