| [Objective]To compare the effects of high tibial osteotomy and unicompartmental knee arthroplasty in the treatment of medial interventricular osteoarthritis of the knee.To provide reference for clinicians and patients to choose appropriate treatment methods.[METHODS]Retrospective analysis of 61 patients with medial interventricular osteoarthritis of the knee treated in the first affiliated hospital of Soochow University from August 2016 to August 2019.There were 30 cases in HTO group and 31 cases in UKA group according to different surgical procedures.The operative time,tourniquet time,intraoperative blood loss,preoperative and postoperative VAS scores,preoperative and postoperative 1 month,3 months,6 months,1 year HSS score and WOMAC score were recorded and compared between the two groups.[RESULTS]The patients in both groups were followed up.HTO group was followed up for an average of 13.5 months,UKA an average of 15.3 months.There was no significant difference in sex,side,age and BMI index between HTO and UKA groups.The VAS score,knee joint HSS score,WOMAC score were(1.70±1.01),(87.50±4.13)and(21.24±2.71),and(1.87±0.84),(88.50±2.23)and(19.39±5.87),respectively.Compared with preoperative,the indexes were significantly improved,and the difference was statistically significant.Particularly in HTO group of cases,the MPTA,of the patients after surgery was significantly corrected,and the lower extremity force line was reconstructed.The difference was statistically significant.There were no significant differences among the two groups in length of operation,tourniquet use time,intraoperative bleeding volume and surgical incision length(P>0.05),while in the mean hospital stay,the HTO group was(10.71 ±3.86)days,while the UKA group was(6.13±0.86)days,the two groups have no statistically differences(P<0.05).There was no significant difference in preoperative knee HSS score(t=0.842,Pt=0.403)and WOMAC score(t=-0.475,Pt=0.637)between the patients in the group and the group(0.05).Similarly,there was no significant difference in postoperative knee HSS score(t=-0.418,P=0.678)and WOMAC score(t=-0.187,0.852)(0.05).CONCLUSIONS]HTO and UKA have obvious therapeutic effect on medial interventricular osteoarthritis of knee joint after selecting patients according to surgical indications.However HTO due to its own technical factors,it is more suitable for younger patients with higher daily activity needs,bone mineral density and better bone mass.UKA is not only an important part of knee arthritis ladder therapy,but also can be used as one of the terminal treatment methods for knee joint OA.Therefore,the choice of the two procedures needs to consider clinical and patient factors. |
