Objective:The purpose of this study to compare the short-term in vitro diaphragmpacemaker(external diaphragm pacing,EDP)with different body mass index,body mass index,BMI)acute exacerbation of chronic obstructive pulmonary disease period(acute exacerbation of chronic obstructive pulmonary diseases,AECOPD)patients in the curative effect of pulmonary function,arterial blood gas analysis,To explore which nutritional status of patients with COPD acute exacerbation phase is more suitable for short-term EDP rehabilitation treatment strategy.Methods:Collected in January 2019-January 2020 the first affiliated hospital ofkunming medical university because of AECOPD,90 cases of hospitalized patients as the research object,the selection of BMI<18.5 into the lower BMI groups,30 patients of 18.5 or less BMI<24.0 patients with normal BMI groups,30 patients in the high BMI 24.0 or higher BMI groups,different BMI group 15 cases were randomly divided into control group and treatment group,15 cases in the control group to the oxygen therapy and anti-infection,cough and asthma,maintain water and electrolyte balance,such as conventional treatment,treatment group with conventional therapy on the basis of combined therapy with EDP,After 10 days of treatment,two groups of patients with different BMI were compared,respectively,with forced expiratory vital capacity in one second expected(FEV 1%),arterial oxygen pressure PaO2,and arterial carbon dioxide pressure.The therapeutic effect of PaCO2).Results:(1)comparison of baseline data between the treatment group and the control group:there was no significant difference in age,sex,smoking course,previous hypertension,diabetes history,body mass index and other basic data between the two groups(P>0.05).(2)FEV 1%,PaO2 in the treatment group were higher than those in the control group,and the differences were statistically significant(P<0.05).No significant difference was found in PaCO2(P>0.05).(3)FEV 1%,PaO2 in the treatment group were higher than those in the control group,and the differences were statistically significant(P<0.05).No significant difference was found in PaCO2(P>0.05).(4)FEV 1%,PaO2 were improved in the treatment group and the control group,but there was no significant difference between the two groups(P>0.05).No significant difference was found in PaCO2(P>0.05).(5)there was no significant difference in pretreatment parameters(fev1%,PaO2,and PaCO2)between the high,normal and low BMI groups(p>0.05).(6)after treatment,fev1%in the high,normal and low BMI groups showed significant differences in PaO2(p<0.05),while>in the high BMI group and low BMI group showed significant differences in PaO2(p<0.05).>low BMI treatment group;There were no significant differences in PaCO2 among the three groups(p>0.05).Conclusions:1.Short-term EDP treatment can improve the intensity and endurance of diaphragm,improve pulmonary function and hypoxia,and promote pulmonary rehabilitation of AECOPD patients.2.Short-term EDP treatment of AECOPD patients with good nutritional status and normal BMI was better than that of patients with poor nutritional status and low BMI,and pulmonary rehabilitation treatment of patients with low BMI on the basis of nutrition enhancement may be more effective.
