Part oneObjective:The purpose of this study was to investigate the effect of serum thyroglobulin antibody(TgAb)and thyroid peroxidase antibody(TPOAb)concentration on lymph node metastasis in patients with papillary thyroid carcinoma.Methods:A retrospective analysis was made of the medical records of 2604 patients with thyroid papillary carcinoma diagnosed pathologically after surgery in the first affiliated hospital of kunming medical university from April 2012 to December 2018,and the clinical data of 2604 patients with thyroid papillary carcinoma diagnosed pathologically after surgery were screened according to the inclusion criteria.Univariate analysis was performed on the relevant factors that may affect the lymph node metastasis in patients with papillary thyroid carcinoma.The meaningful variables were combined with the concentrations of serum thyroid autoantibodies TgAb and TPOAb for further multivariate logistic regression analysis to determine the serum TgAb and TPOAb concentrations the relationship with lymph node metastasis in patients with papillary thyroid carcinoma.Results:Through the single factor analysis of selected in addition to the concentration of serum TgAb,TPOAb may affect patients with thyroid papillary carcinoma metastasis of lymph node in central region factors:gender(p<0.001),age(p<0.001),tumor size(p<0.001),multiple focal(p<0.001),if there is a gland outside infiltration(p=0.011),whether infringement of blood vessels(p<0.001),the total number of lymph node in central-region(p<0.001),serum TSH concentrations(p=0.016).These factors,serum TgAb concentration and serum TPOAb concentration were included in the multivariate analysis,and the results showed that serum TPOAb concentration in patients with thyroid papillary carcinoma was significantly correlated with lymph node metastasis in central region(p<0.01).Patients with higher serum TPOAb had a lower lymph node metastasis rate in central region.Serum TgAb concentration was not associated with lymph node metastasis in the central region(p=0.747).Conclusion:Our research once again proved that males,young age,large tumor diameter,multifocal tumors,tumor invasion and capsule,tumor invasion and blood vessels,and central lymph nodes are the independent risk factors for the metastasis of papillary thyroid carcinoma.Tumor invasion to the capsule,tumor invasion to nerves,and serum TSH concentration were not associated with lymph node metastasis in patients with papillary thyroid carcinoma.Serum TPOAb concentration in patients with papillary thyroid carcinoma without central lymph node metastasis was significantly higher than that in patients with central lymph node metastasis,the difference was statistically significant.Serum TgAb concentration in patients with papillary thyroid carcinoma is not related to lymph node metastasis in the central area.Part twoObjective:The purpose of this study was to investigate the expression and significance of thyroid peroxidase(TPO)in papillary thyroid carcinoma and central lymph nodes.Methods:Collected in July 2019 to December 2019 in the first affiliated hospital of kunming medical university thyroid surgery surgical treatment of the patient's medical records,in accordance with the filtered into the drainage standard after collected a total of 55 patients with thyroid papillary carcinoma and 37 cases of pathological specimens of lymph node in central region,divided into thyroid papillary carcinoma tissue,thyroid papillary carcinoma in normal tissue adjacent to carcinoma group,the negative group and positive group of lymph node in central region,and lymph node in central region with the method of immunohistochemical detection of TPO expression in different tissue,the results are half quantitative analysis,comparative differences between groups,combines the condition of lymph node metastasis,The differences and significance of TPO expression in different groups were analyzed.Results:The positive expression rate of TPO in papillary thyroid tissue was low,only 12.7%,and the degree of positive expression was low,all of which were weak positive expressions.The positive expression rate of TPO in normal tissues adjacent to papillary thyroid carcinoma was 100%,and the expression intensity was strong.TPO was not expressed in the negative lymph node group,and the expression rate of TPO in the positive lymph node group was 10.8%,and the expression was not different from the primary tumor.Conclusion:The expression rate of TPO in papillary thyroid carcinoma is much lower than that in normal tissues adjacent to the cancer.TPO is not expressed in normal central lymph nodes that have not metastasized to thyroid papillary carcinoma.The expression rate of TPO in central lymph nodes metastasized by papillary thyroid carcinoma was no different from that of cancer.The expression of TPO in papillary thyroid carcinoma and central lymph nodes was not related to central lymph node metastasis.
