[Objective]Combined detection of TBNK lymphocyte subsets,12 cytokines,HLA-DR positive expression rates and neutrophils in patients with bloodstream diseases, bloodstream infections, other non-bloodstream infections and uninfected patients Cell CD64 infection index and other indicators, collect all the indicators in mIPS and quantify the scores, based on the above five indexes,an infection evaluation system is constructed to evaluate the changes of infection indexes when patients with blood system diseases develop bloodstream infections and evaluate the feasibility of the system..[Methods]This study included 26 patients with hematological diseases(experimental group)with bloodstream infections,22 patients with other hematological diseases with non-bloodstream infections(control group ?),and 20 patients who were returned to hospital without infection.Patients with hematological diseases(control group II)and collected peripheral blood of each patient separately.Flow cytometry was used to detect TBNK lymphocyte subsets,12 cytokines,HLA-DR positive expression rate of monocytes, and CD64 infection index of neutrophils in the specimens.Clinical data in mIPS were collected and quantified.[Results]1.The mIPS score of the experimental group[(21.002.00),n=26]was significantly higher than that of the control group ?[(16.008.00),n=22],the difference was statistically significant(P<0.001);the experimental group The mIPS score[(21.002.00),n=26]was significantly higher than the control group ?[(0.005.00),n=20],the difference was statistically significant(P<0.001);the control group ? mIPS score[(16.00 8.00)]was significantly higher than the control group ?[(0.00 5.00)],the difference was statistically significant(P<0.001)2.According to a=0.05 level, the experimental group compared with the control group ? more Cytokines(IL-6,IL-8,IL-10,TNF-?)were significantly increased, the difference was statistically significant(P value see details in the text);the experimental group compared with the control group ? multiple cytokines(IL-1?,IL-2,IL-6,IL-8,IL-10,TNF-?)were significantly increased, the difference was statistically significant(P value see the text);Control group ? compared to the control group Multiple cytokines?(IL-1?,IL-2,IL-6,IL-8,IL-10,TNF-?)were significantly increased, the difference was statistically significant(for details of P value, please refer to the text)3.Press a=0.05 level, the absolute value of multiple lymphocytes(CD3+T cell absolute value,CD4+T cell absolute value,CD8+T cell absolute value,NK cell absolute value,CD 19+B absolute value,CD4/CD8)significantly reduced, the difference was statistically significant(see P text for details);the absolute value of multiple lymphocytes in the experimental group was significantly lower than that of the control group ?,and the difference was statistically significant(see P text for details));The absolute value of multiple lymphocytes in the control group ? was significantly lower than that in the control group ?,the difference was statistically significant(for the P value, see the text)4.The CD64 infection coefficient of neutrophils in the experimental group[(0.81 0.35),N=22]was significantly lower than the control group ?[(3.180.47),n=22],the difference was statistically significant(P<0.001);the neutrophil CD64 infection coefficient of the experimental group[(0.810.35),n=26]was significantly lower than the control group ?[(1.86 0.18),n=20],the difference was statistically significant(P<0.001);the control group ? neutrophil CD64 infection coefficient[(3.18 0.47),n=22]was significantly higher than that of control group ?[(1.86 0.18),n=20],the difference was statistically significant(P<0.001);5.HLA-DR positive expression rate of monocytes in experimental group[(36.18 5.67),n=26]was significantly lower than the control group ?[(49.354.37),n=22],the difference was statistically significant(P<0.001);experiment The HLA-DR positive expression rate of monocytes in the group[(36.455.48),n=26]was significantly lower than that in the control group ?[(66.007.25),n=20],the difference was statistically significant(P<0.001);The positive expression rate of HLA-DR in control group ? monocytes[(49.00 7.9),n=22]was significantly lower than that in control group ?[(66.00 7.25),n=20],the difference was statistically significant(P<0.001).[Conclusion]Through combined flow cytometry and mIPS for predicting and analyzing blood flow infection in hospitalized patients with hematological diseases,it was found that three indicators including joint detection of 12 cytokines,TBNK lymphocyte subsets and HLA-DR positive expression rate of monocytes can be used for preliminary Construct a comprehensive evaluation system for bloodstream infection for bloodstream infections in order to detect the occurrence of bloodstream infections in patients with hematological malignancies in hospitals. The mIPS table needs to be further optimized to include indicators. The neutrophil CD64 infection coefficient is not suitable for predicting bloodstream infections in patients with granulocytosis...
