Risk Factors Analysis Of Necrotizing Enterocolitis In Twin Preterm Infants

Objective To analyze the risk factors of neonatal necrotizing enterocotis(NEC)in premature twins.Methods Retrospective analysis was performed on the basis of twin matching from January 1,2015 to June 30,2019 born in the first affiliated hospital of anhui medical university.Clinical data of premature twins with NEC in only one of the twins,the babies were divided into the NEC group and the control group according to whether they had NEC during hospitalization,compared the clinical data of two groups of children,including sex,birth,body quality,Apgar score,perinatal risk factors and complications,treatment and feeding conditions,and so on.Results Fifty-five pairs of twins were collected and 45 pairs of premature twins were included in the study.Univariate analysis showed that there were significant differences in antibiotic use,premature rupture of membranes,less than gestational age,hypoproteinemia,albumin infusion,feeding intolerance,and neonatal sepsis,which were associated with NEC(P<0.05).Conditional logistic regression analysis showed that antibiotic use(P=0.006,OR=3.175,95%CI: 1.452~11.049),premature rupture of membranes(P=0.029,OR=2.711,95%CI: 1.598~8.850),SGA(P=0.003,OR=4.266,95%CI: 2.481~13.294),feeding intolerance(P=0.011,OR=4.134,95%CI:3.556~7.982)and neonatal sepsis(P=0.025,OR=3.188,95%CI: 1.246~6.133)are the main risk factors of NEC.Conclusions The use of antibiotics,premature rupture of membranes,infants younger than gestational age,feeding intolerance and neonatal sepsis are related to the incidence of NEC.When children are combined with the above risk factors,clinicians should pay more attention to early detection and intervention.
