| Objective: To analyze the high risk factors of neurological complications after type A aortic dissection.Methods:120 patients with type A aortic dissection received in our hospital from January 2017 to June 2019 were selected as the study objects,according to whether the neurological complications were divided into the group(n =31)and the group without complications(n =89),the proportion of the type of postoperative neurological complications was analyzed,and the clinical characteristics of the two groups were compared,the operation situation,and the postoperative complications factors of type A aortic dissection were analyzed by univariate and multifactor logistic regression analysis.Results:(1)The clinical characteristics of the two groups were analyzed,and the comparison between the groups showed that there was no statistically significant difference between the patients in the complications group and the patients without complications in terms of gender,age,and disease incidence(P> 0.05).(2)Among the 120 patients,31 patients had neurological symptoms,the incidence rate was 25.83%,24 patients had transient neurological impairment(20.00%),and 7 had permanent neurological impairment(5.83%).Thirty patients presented with neurological symptoms before surgery,of which 5 patients showed chest pain symptoms,3 patients had left limb hemiplegia,and 2 patients had leaf limb numbness and weakness pain.The types of neurological complications in 31 patients were as follows;9 patients had ischemic stroke(29.03%),2 patients had cerebral hemorrhage(6.45%),6 patients had ischemic neuropathy(19.35%),8patients had a nerve compression syndrome(25.81%)and 6 patients had hypoxic encephalopathy(19.35%).(3)Analysis of the surgical conditions of the two groups,the comparison results between the groups showed that the incidence of hypertension in the complications group was significantly higher than that in the non-complication group,and the data difference was statistically significant(P <0.05);Significantly higher than those in the non-complication group,the data difference was statistically significant(P<0.05).(4)After multivariate logistic regression analysis,hypertension,operation time,extracorporeal circulation time,aortic occlusion time,ventilator assisted breathing time,ICU staying,time of cadiac arrest,cerebral perfusion time are independent effects of postoperative neurological complications in patients with type A aortic dissection.Factors(P <0.05).Conclusion: hypertension,operation time,extracorporeal circulation time,aortic blocking time,ventilator assisted respiratory time,ICU staying,time of cadiac arrest,cerebral perfusion time are the influencing factors of postoperative neurological complications in patients with type A aortic dissection. |
