Evaluation Of Early Efficacy Of Ultrasound-guided Percutaneous Transluminal Angioplasty Intreatment Of Arteriovenous Fistula Stenosis

ObjectiveTo investigate the early effect of ultrasound-guided inferior intravascular angioplasty in the treatment of hemodialysis arteriovenous internal fistula(AVF)stenosis.MethodsA retrospective cohort study was conducted to collect the clinical data of 24 dialysis patients with AVF stenosis in the forearm who received ultrasound-guided PTA treatment in our hospital from January 2017 to December 2018,including gender,age,operation time,postoperative follow-up time and patency,postoperative complications,and internal diameter of the stenosis before and after surgery.Survival risk factors and vascular patency rate were studied by kaplan-meier survival analysis.ResultsAll the 24 patients were successful with 100% success rate,no pseudoaneurysm or subcutaneous hematoma occurred,and no death occurred during the perioperative period,with a complication rate of 4.17%(1/24),except for thrombosis after AVF angioplasty in 1 patient.The mean internal diameter of vascular fistula stenosis before and immediately after surgery was(2.06±0.73)mm and(3.62±0.88)mm,respectively,the difference was statistically significant(t=9.84,P<0.05).The primary patency rates at 3,6,9 and 12 months were 87.50%(21/24),83.33%(20/24),79.17%(19/24)and 58.33%(14/24),respectively.Conclusions1.PTA is an effective treatment for recurrent stenosis and occlusion after AVF surgery,which can effectively prolong the use time of internal fistula.2.After angioplasty,the early patency rate of arteriovenous fistula was significantly improved,but showed a decreasing trend with time.3.UG-PTA is safe,effective,economical and simple for the treatment of postoperative stenosis of AVF,and can improve the success rate of surgery.It is a reasonable treatment method for stenosis of AVF,with obvious short-term effect.
