Objective:To explore the correlation between vulnerable plaque and prognosis of carotid artery stenting(CAS)in patients with clinical symptomatic carotid stenosis,thus providing an effective basis for clinical prognosisMethods:A total of 127 patients with symptomatic carotid stenosis who were admitted to the Department of Neurology at the Fourth Affiliated Hospital of China Medical University from September 2017 to September 2019 were selected and all were given standardized medical treatment and carotid artery stenting.Collect basic clinical data of the patient,including general information,pre-morbid mRs score,smoking history,drinking history,past history,admission-related test results(blood lipids,blood glucose,glycated hemoglobin,homocysteine),preoperative head and neck CTA Image data,postoperative smoking and alcohol withdrawal,vascular lesions,surgical timing,and follow-up within 3 months after surgery.In this study,the 127 patients were divided into 2 groups by the mRS score at 3 months after surgery,of which 9 were lost to follow-up for various reasons:good prognosis group(mRS score 0?2,a total of 90 cases),and poor prognosis group(mRS A total of 28 cases were scored from 3 to 6 points).Through retrospective analysis,the differences between the two groups were compared.The measurement data was analyzed by t test or u test,the count data was analyzed by chi-square test,and the relationship between vulnerable plaque and prognosis of symptomatic carotid stenosis stenting was further analyzed by multivariate logistic regression analysis.P<0.05 suggested that the data were statistical significanceResults:Compared with the good prognosis group,there were 28 patients in the poor prognosis group,18(62.28%)patients with hyperlipidemia,19(67.85%)patients with hypertension,and 22(78.57%)vulnerable plaques.0.05,there is a statistical difference.There was no significant difference in the timing of operation,type of stent,side carotid artery,side carotid artery stenosis rate,and postoperative adverse reaction between the two groups,with P values greater than 0.05.The results of multivariate logistic regression analysis of hyperlipidemia,hypertension,and vulnerable plaques showed:hypertension(OR=2.62,95%Cl:1.036?6.63,P=0.042),vulnerable plaques(OR=3.684,95%CI:1.373?9.889,P=0.010)The difference was statistically significant.Conclusion:1.Hyperlipidemia,hypertension,and vulnerable plaque are risk factors for poor prognosis of patients with symptomatic carotid stenosis.Among them,vulnerable plaque and hypertension are independent risks of poor prognosis factor.2.Vulnerable plaque is an important index for predicting the prognosis of symptomatic carotid stenosis after CAS,and has a good correlation with the prognosis Patients with symptomatic carotid stenosis with stable plaque may have a better clinical prognosis after CAS.
