Analysis Of Segmental Lymph Node Metastasis And Clinical Features In CT1N0M0 Lung Adenocarcinoma

Purposes: Lymph node metastasis is the main pathway for progression of lung adenocarcinoma and segmental lymph node(LSN)metastasis in cT1N0M0 stage lung adenocarcinoma is still unclear.We want to discovery the law of lymph node metastasis between different segments of the early stage of lung adenocarcinoma to contribute to the indications of segmentectomy。 Method: A number of 200 patients through the PET-CT,diagnosed with cT1N0M0 lung adenocarcinoma were enrolled and received lobectomy and lymph node dissection.Sorting lymph nodes in each station.Then we analysised the metastatic status of the iLSNs combined with several characteristics Results: Patients without N1 lymph nodes metastasis(P =0.036),ground glass nodule(P =0.025),AIS/MIA/LPA(P =0.038),nodules with maximum diameter smaller than 1cm(P =0.017),SUVmax <2.5,and serum CEA level(≤4.5 ng/ml)(P = 0.036)had a lower iLSN metastasis rate.Conclusions: Segmentectomy might be a better choice for patients with pure ground glass nodules with CEA no more than 4.5ng/ml,SUVmax <2.5(P =0.029),and the size is no more than 1cm,whose frozen pathology is confirmed as AIS/MIA/LPA.But during the operation,when metastasis to the N1 lymph nodes,segmentectomy would not be suitable.
