Objective: To compare the density of embolism and prognosis of intracranial tiny aneurysms with AxiumTM Prime coil and AxiumTM ordinary coil, and to compare the difference in safety and the prognosis between the two groups. Methods: Patients with small intracranial aneurysms?diameter ? 5mm?admitted to the neurosurgery department of shengjing hospital affiliated of China medical university from March 2017 to October 2018 were collected for endovascular treatment.A total of 136 patients were included in this study,Among them,39 cases of intracranial tiny aneurysms were treated with Prime coil,97 cases of intracranial tiny aneurysms were treated with common coil.Comparing two groups of general data, the degree of immediate aneurysm embolization of the two groups was evaluated by Raymond grading, the perioperative complications of the two groups were compared, and the results of angiography review and postoperative mRS score of the two groups were compared one year after the operation. Results: A total of 136 patients were included in this study, including 39 in the Prime coil group and 97 in the common coil group. There were no statistically significant differences in the data?age, gender, aneurysm location?between the two groups. The Raymond grading of the two groups was compared by immediate postoperative angiography: 36 cases of grade 1,3 cases of grade 2,and 0 cases of grade 3 in the Prime coil group, with an aneurysm occlusion rate of 92.3%;75 cases of grade 1,18 cases of grade 2,and 4 cases of grade 3 in the common coil group, with an aneurysm occlusion rate of 77.3%;the comparison between the two groups was statistically significant?P=0.038 < 0.05?.Among the perioperative complications in the two groups,10 patients in the Prime coil group had treatment-related complications.A total of 17 patients in the general coil group had treatment-related complications. One year after surgery, angiography and postoperative mRS score follow-up were performed on the patients in the two groups. In the Prime coil group,Raymond grade 1 included 35 patients, grade 2 included 2 patients, and grade 3 included 0 patients. The aneurysm occlusion rate was 94.6%.In the common circle group, there were 89 cases of Raymond grade 1,5 cases of Raymond grade 2,and 3 cases of Raymond grade 3,and the aneurysm occlusion rate was 91.7%.There was no significant difference between the two groups?P=0.555 > 0.05?.One year after the operation,mRS score was used to evaluate the prognosis of patients in the two groups.36 patients in the Prime group had a good prognosis, and 87 patients in the ordinary circle group had a good prognosis, with no statistical significance?P =0.352 > 0.05?.Conclusion: The AxiumTM Prime coil is softer, puts less pressure on the aneurysm wall, and has the ability to "drill" better, allowing the coil to be more evenly distributed within the aneurysm, increasing the rate of tight embolization, reducing the risk of intraoperative rupture, and reducing the risk of recurrence.
