Objective:The DNA microarray chip method was used to detect the resistant gene of Mycobacterium tuberculosis,which provided experimental basis for clinical diagnosis and treatment.Research Methods:Collect and organize patients with sputum smear-positive tuberculosis admitted by the TB Department from July 1,2018 to December 31,2019,while traditional sputum mycobacterium tuberculosis culture and drug sensitivity test,DNA microarray chip method detects the resistance of mycobacterium tuberculosis,and both have positive results to reply to patients.All patients were admitted to hospital and left 2 copies of morning sputum to be sent for examination for traditional sputum tuberculosis mycobacterium culture and drug sensitivity,DNA microarray chip method testing.Traditional culture and drug sensitivity were used to improve Roche culture and preliminary identification of strains,and parallel proportional method anti-tuberculosis drug sensitivity tests.The results of traditional culture and drug sensitivity test sit as the gold standard.the dna microarray chip method compared the monitoring results of rifampin resistance with traditional mycobacterium tuberculosis culture and drug sensitivity tests,compared the results of multidrug resistance test results,and summarized the mutation of gene sites.Results:1.Comparing the sample results of 416 patients selected,with traditional culture and drug sensitivity test results as the gold standard,the sensitivity of DNA microarray chip method to detect rifampin resistance was 84.31%,the specificity was 99.18 %,the consistency rate was 97.36 %,the positive prediction was 93.48 %,and the negative prediction was 97.84 %.X2 is 1.45,P.05.The difference between the two methods in the monitoring results of Lifupin resistance was not statistically significant.2.We detected 46 gene mutations in DNA microarray,including 43 cases of true resistance and 3 cases of false resistance.In the true drug resistance,36 cases had rpoB531 mutation,the mutation rate was 78.26 ? 3 ?;4 cases had rpoB 526 mutation,1 case had rpoB 516 mutation,1 case had rpoB 511 and rpoB 516 mutation at the same time,1case had rpoB 511 and rpoB 526 mutation at the same time.Among the 3 cases with pseudodrug resistance,1 case had rpoB 516 mutation and 2 cases had rpoB 511 mutation.|3.The sample results of 416 selected patients were compared with the traditional culture and drug sensitivity test results as the gold standard,and the sensitivity of THE DNA microarray chip method to detect multidrug-resistant results was 89.29 percent.The specificity is 80.00 percent,the consistency rate is 81.67 percent,the positive prediction is 80.65 percent,and the negative prediction value is 88.89 percent.Conclusion:By evaluating the effect of DNA microarray chip method to detect the resistance gene of Mycobacterium tuberculosis,it is proved that the detection method has high sensitivity,and has a high rate of consistency with the traditional mycobacterium tuberculosis culture drug sensitivity test.Moreover,the detection time of this method is short,the results are reliable and the price is relatively economical,and it is worth applying to the diagnosis of clinical tuberculosis resistance and to guide the adjustment of drug resistance scheme.The development of a DNA microarray chip kit covering all known mutation sites of TB drugs will further expand the scope of application of this method,and the results will be more reliable if the sensitivity to drug-resistant strains can be improved.
