Objectives : To investigate diagnostic value the evaluation of lymph node metastasis and lymphovascular space invasion(LVSI)in cervical cancer patients by radiomics signature based on T2-weighted imaging fat suppression(T2WI-FS)and Apparent diffusion coefficient(ADC).Materials and methods:Rtrospectively analysis 77 patients with surgical pathology proven cervical cancer,with paitents clinical informations,pathology features,axial T2WI-FS and ADC(b=0/900 s/mm2)images,of the First hospital of Lan Zhou University.Imaging was standardized preprocessed by A.K.software,ROI were performed on preprocessing imaging with manual segmentation in ITK-SNAP software.The preprocessed and ROI images of all patients were imported into A.K.software,and 402 radiomic features of each lesion were extracted on T2WI-FS and ADC respectively.Radiomic features were screened by ANOVA+MW combining LASSO regression mothods.The clinical characteristics with lymph node metastasis and LVSI were identified based on the t-test for continuous variables,rank data for Wilcoxon rank sum test and the chi-square test for categorical unordered variables in SPSS 25.0 software.Besides,logistic regression model was established for the radiomics signatures,the diagnostic performance of each radiomics signatures were evaluated by using Med Calc software to draw the receiver operating characteristic(ROC)curve.Results: 1.There was a association between the depth of cervical stromal invasion with LVSI and lymph node metastasis(P<0.05).Besides,LVSI and lymph node metastasis with strongly correlated(P = 0.001).2.The radiomics signature for T2WI-FS,ADC,T2WI-FS combined ADC models evaluate lymph node metastasis of AUC were 0.850(95% CI:0.751~0.921),0.887(95%CI:0.787~0.951),0.935(95% CI:0.848~0.981),respectively.3.The radiomics signature for T2WI-FS,ADC,T2WI-FS combined ADC models evaluate LVSI of AUC were 0.816(95% CI:0.712~0.895),0.850(95% CI:0.747~0.923),0.899(95% CI:0.805~0.957),respectively.Conclusion: 1.The depth of cervical stromal invasion is a risk factor for the presence of lymph node metastasis and LVSI.Besides,LVSI and the presence of lymph node metastasis with strongly correlated.2.Radiomics signature based on T2WI-FS?ADC and multi-sequence combined can evaluate the lymph node metastasis and LVSI in cervical cancer.Besides Multisequence combined radiomics signature model with the best diagnostic efficiency,the diagnostic performance of ADC radiomics features showed better compared with T2WI-FS.
