Prognostic Risk Factors In Patients Undergoing Radical Cholecystectomy For Gallbladder Cancer

?Objectives? To investigate the prognostic factors affecting patients undergoing radical surgery for gallbladder cancer by retrospective analysis,and thus to provide guidance for clinical management of gallbladder cancer.?Methods? The clinical data of patients with gallbladder cancer who underwent radical surgery in Zhongda Hospital Affiliated to Southeast University from March 2010 to March 2018 were collected.Uni-and multi-variable Cox regression analysis was utilized to identified the independent statistically prognostic factors for OS(overall survival).ROC analysis was used to compared predictive accuracy between different risk factors for OS.?Results? The 1-,2-and3-year survival rate of 50 gallbladder cancer patients undergoing radical surgery are 76%,55%,37.6%.Univariable Cox regression analysis showed that CEA,CA19-9,NLR,SIS,GPS and GBRS were potential significant prognostic factors(P<0.05);Time dependent ROC curve analysis found that the area under the SIS curve was significantly greater than the GPS at postoperative 1 year(AUC: SIS=0.748,GPS=0.603,P=0.024);Subgroup analysis found in advanced patients,SIS score was statistically significant(P = 0.014);Cox multivariate analysis showed that CA19-9(Hazard ratio HR=2.812,P=0.005),SIS score(HR=2.072,P=0.014)and GBRS score(HR=2.948,P=0.004)were independent prognostic risk factors.?Conclusions? 1.SIS is superior to GPS for predicting OS in patients with gallbladder cancer who underwent radical surgery.2.CA19-9,SIS and GBRS are independent risk factors for prognosis of patients with gallbladder cancer,they are easy to obtain and should be evaluated routinely before and after surgery.
