Objective 1.To investigate the status of type D personality,chronic illness resources utilization,perceived control and empowerment in patients with coronary heart disease(CHD).2.To investigate the influencing factors of empowerment in patients with CHD.3.To explore the mechanism of type D personality affecting empowerment in patients with CHD,and analyze the mediation effects of chronic illness resources utilization and perceived control in between,so as to provide a theoretical basis for developing personalized interventions for improving empowerment of patients with CHD.Methods this study was a cross-sectional study.From May 2019 to November 2019,319 patients with CHD who were hospitalized from the department of cardiovascular medicine of a tertiary hospital in Hefei City,Anhui Province were selected for questionnaire survey,using convenient sampling method.The general information questionnaire,Type D Personality Scale(DS-14),Chinese version of Chronic Illness Resource Survey Scale(CV-CIRS),Control Attitudes Scale-Revised(CAS-R)and Chinese version of patient perception empowerment scale(CV-PPES)were used to investigate the general condition,type D personality,perceived control,chronic illness resources utilization and empowerment of CHD patients.Statistical analysis was performed using SPSS 21.0 and PROCESS 3.0 plug-ins.Results Among the 319 patients,115 were type D personality,accounting for 36.1%,the score of chronic illness resources utilization was(43.02±10.16),the score of perceived control was(24.19±6.70),and the empowerment score is(33.99±7.20).Single factor analysis results showed that the empowerment of CHD patients was related to religion,education level,occupation,family residence,medical security,disease cognition and family history of CHD,with statistically significant differences(P<0.05);Correlation analysis showed in patients with CHD,type D personality was negatively associated with chronic illness resources utilization(r=-0.253,P<0.01),perceived control(r=-0.587,P<0.01)and empowerment(r=-0.343,P<0.01),chronic illness resources utilization was significantly positively related to empowerment(r=0.567,P<0.01),perceived control was positively associated with empowerment(r=0.410,P<0.01),chronic illness resources utilization was positively associated with perceived control(r=0.404,P<0.01);Multiple regression results showed that religion,type D personality,perceived control,medical staff support,neighborhood or community support,and media and policy support had predictive effects on empowerment of patients with CHD,and the combined interpretation variance was 53.4%.The mediating analysis showed that both chronic illness resources utilization and perceived control partially mediated the effect of type D personality on empowerment,which accounted for 35.2% and 20.1% of the total effect respectively.The chain mediating effect of chronic illness resources utilization and perceived control between type D personality and empowerment accounted for 2.6% of the total effect.Conclusions 1.The distribution of type D personality in patients with CHD conforms to the domestic norm,and chronic illness resources utilization,perceived control and empowerment of chronic diseases are all at a low level.2.Religion,type D personality,perceived control,medical staff support,neighborhood or community support,and media and policy support are influencing factors of empowerment of patients with CHD;3.The chronic illness resources utilization and perceived control of CHD patients have partial mediating effect between type D personality and empowerment,while the chronic illness resources utilization and perceived control also have chain mediating effect between the two.
