Objective to investigate the effect of diltiazem in percutaneous coronary intervention(PCI)on early cardiac function in patients with acute myocardial infarction(AMI).Methods A total of 107 patients with acute myocardial infarction who underwent PCI in the department of cardiology of our hospital from January 1,2018 to December 30,2018 were selected.The patients were divided into 54 patients in the control group and 53 patients in the experimental group by the random number table method.In the control group,8ml saline was injected in the distal part of the lesion within 3-5minutes after the perforated balloon was passed through the criminal vessel.Patients in the experimental group were given 8ml diltiazem(125ug/ml)injection within 3-5minutes.Patients in the two groups were followed up for six months.Compare two groups of patients with cardiac function index(including preoperative and postoperative 24 hours of creatine kinase isoenzyme(CK-MB),troponin I(CTn I)and amino terminal brain natriuretic peptide precursor(NT-pro BNP),PCI postoperative TIMI flow grade,TIMI myocardial perfusion(TMP)classification,and PCI postoperative 1 week and 6months after left heart ejection fraction(EF),left ventricular shortening(FS)and left ventricular end-diastolic diameter(LVEDD),and two groups of patients in hospital and postoperative half year major adverse cardiovascular events(MACE).Results After treatment,there were 49 cases of TIMI blood flow grade 3 and 42 cases of TMP blood flow grade 3 in the experimental group,and 42 cases of TIMI blood flow grade 3 and 32 cases of TMP blood flow grade 3 in the control group,with statisticallysignificant differences(P<0.05).Ck-mb,CTn I and nt-probnp were better in the experimental group than in the control group 24 hours after surgery,P<0.05.EF(58.32±7.10%),FS(37.09±5.56%)and LVEDD(47.39±4.46mm)in the experimental group,EF(42.05±5.67%),FS(29.03±9.51%)and LVEDD(48.81±6.16mm)in the control group were significantly different(P<0.05).Six months after the operation,there were 1 case of sudden death and 5 cases of arrhythmia in the control group,and no adverse cardiovascular events occurred in the experimental group(P<0.05).Conclusion Diltiazem can improve TIMI blood flow and TMP blood flow grading in patients with AMI through percutaneous coronary intervention,and EF and FS are significantly better than before surgery,so it has certain effect on improving early cardiac function of patients.
