Objective: To explore the clinical value of 3D-T2 WI Turbo Spin-echo(VISTA)in the preoperative diagnosis of cervical cancer,especially in the periuterine infiltration.Methods: 142 patients with stage IB-IIB cervical cancer were selected for the study.The clinical data were complete.Among them,71 patients had conventional 2D MRI sequence,and 71 patients had VISTA sequence.The imaging results,clinical examination and postoperative pathology of 142 patients with cervical cancer were analyzed retrospectively,and the three staging results were compared.Results: Compared with the pathological results,the sensitivity of clinical examination,routine sequence imaging and VISTA imaging in the diagnosis of IB stage was 90.9%,93.2% and 95.5%,the specificity was 51.9%,81.5% and 88.9%,the accuracy was 75.5%,89.1% and 93.3%,and the sensitivity in the diagnosis of IIA stage was 92.3%,84.6% and 92.3%,respectively,The specificity was 92.2%,98.3%,98.3%,and the accuracy was 72.7%,91.7%,92.3%,respectively.The sensitivity and specificity of routine MRI sequence and VISTA were 78.6%,85.7%,98.2%,98.2% and Youden index were 76.8%,83.9% respectively.Through kappa test on VISTA imaging and postoperative pathological data,kappa = 0.776(P < 0.01),and McNemar Bowker test,P = 0.189,there was no significant difference between them.The kappa value of conventional MRI sequence and postoperative pathological results was 0.565(P < 0.01).P < 0.05,which was statistically significant.Conclusion: The Pelvic MRI VISTA sequence is more accurate for the preoperative staging of stage IB-IIB cervical cancer than the conventional 2D MRI sequence,The Pelvic MRI VISTA sequence is more accurate than the conventional 2D MRI sequence in the preoperative staging of stage IB-IIB cervical cancer,and it is more accurate and sensitive than the conventional MRI sequence in the preoperative diagnosis of peri uterine invasion of cervical cancer,which can provide a more objective scientific basis for the preoperative diagnosis and staging of cervical cancer.
